Prior to 2008, all personnel that were monitored for occupational exposure received these. As our exposure records are administered through the campus Environmental Health and Safety (EH&S) office, they receive our exposure results from the contractor and then pass them on to us. When questioned as to why we did not receive any Form 5's for 2008, we were told that they no longer issued them to everybody, only those individuals that receive a dose in excess of 10 percent of the limits of 20.1201(a), or 500 mrem.

Although this is true, the same section also states that individuals entering high radiation areas also require this report. This was not noted at the time, and since we haven't had any individual receive above 500 mrem in one year, we did not receive any form 5's.

(2) Corrective steps that have been taken and results achieved: The campus EH&S office has been notified to issue all MNRC occupational workers Form 5's in the future. We are told we will be receiving them for at least the last three years (2009, 2010, and 2011).

(3) Corrective steps that have been taken to avoid further violations: An annual item has been added to the Preventive Maintenance database to request these forms.

(4) Date when full compliance will be achieved: Full compliance will be achieved when the Form 's are received from the contractor, no later than 31 December 2012.
