Report on Inoperable Main Steam Line Radiation Monitor Indian Point Unit No. 3 Docket No. 50-286 License No. DPR-64

Dear Sir or Madam:

The purpose of this letter is to submit a report pursuant to Technical Specification (TS) 5.6.7.

One Radiation Monitor channel per main steam line is required to be operable in Modes 1, 2, and 3 as per TS 3.3.3, Table 3.3.3-1, item 22. Radiation monitor R-62A (Main Steam Line Radiation Monitor) was declared inoperable on October 17, 2013. TS 3.3.3 Condition A requires the radiation monitor to be fixed within 30 days, and if this completion time is not met, Condition B requires a report be submitted to the NRC within the next 14 days pursuant to TS 5.6.7. The 30 days expired on November 17, 2013. Pursuant to TS 5.6.7, the report is to outline the alternate method of monitoring, the cause of the inoperability, and the plans and schedule for restoring the instrumentation channels of the function to operable status.

  • Alternate method of monitoring The alternate method of monitoring for an out of service Steam Line Radiation Monitor channel is via Condenser Air Ejector radiation monitor R-1 5 and the Steam Generator Blowdown Monitor R-19. With these radiation monitors in service, no compensatory sampling is required.
  • Cause The cause of the inoperability is the failure of the channel 1 (R-62A) High Voltage Power Supply.

NL-13-148 Docket 50-286 Page 2 of 2 Plans and schedule for restoring The current plan to restore the channel to operable is to procure a new power supply from the original vendor (General Atomics). The schedule for the new power supply to be on site is during the week of November 25, 2013 and the anticipated installation and testing should restore the channel to operable no later than 12/13/2013.
