Title | Failure to Implement and Establish Appropriate Work Instructions Affecting Safety-Related Chiller |
Description | A Self -Revealing, Green, non- cited violation (NCV) of Technical Specifications (TS) 5.4.1.a, Procedures, was identified for the licensees failure to implement maintenance work instructions and establish appropriate procedures concerning the use of flow measurement and test equipment (M&TE) in support of essential safety features (ESF) chilled water pumps in- service testing (IST). As a result, the Unit 1 A train safety -related chiller was inadvertently rendered inoperable when technicians isolated a flow transmitter associated with the chillers auto -start control logic when installing and removing M&TE in support of the IST. The licensee entered this issue into their corrective action program (CAP) under condition report (CR) 10390340 and corrective action report 270610 and planned to revise the procedure. Failure to implement maintenance work instructions and establish appropriate procedures concerning the use of flow M&TE in support of ESF chilled water pumps IST, which can affect ESF chiller performance, as required by Regulatory Guide (RG) 1.33, Quality Assurance Program Requirements, of February 1978, was a performance deficiency (PD). The performance deficiency was more than minor because it was associated with the Equipment Performance attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. The finding was determined to be of very low safety significance (Green) because while the unit 1 A train ESF chiller was rendered inoperable, it did not represent a loss of function of the train for greater than its TS Allowed Outage Time. The finding was assigned a cross cutting aspect of Challenge the Unknown because questions and risks regarding the use of flow M&TE for the test were not properly evaluated and managed before proceeding.
