Review Turbine-Driven Auxiliary Feedwater Pump Mechanical Overspeed Trip Device out of Specification

Description

The team determined that the out-of-specification tappet nut to head lever engagement of the turbine-driven AFW pump mechanical overspeed trip mechanism warranted additional NRC review and follow-up considering that this maintenance deficiency led directly to the spurious overspeed trip following the LOOP event and additionally might have affected the reliability of the turbine during a seismic event. Additional review by the NRC will be needed to determine whether this issue represents a performance deficiency. The issue will be identified as URI 05000482/2012008-03, Review Turbine- Driven Auxiliary Feedwater Pump Mechanical Overspeed Trip Device Out of Specification.
