| Title | Failure to Maintain Command and Control During a Feedwater Control Valve Malfunction |
| Description | While reviewing the licensee response to a Unit 3 feedwater pump trip, reactor cutback, reactor trip, and main steam isolation system actuation on June 27, 2018, the inspectors identified that the licensee did not meet the command and control standards outlined in station Procedure 40DP-9OP02 Conduct of Operations, Revision 72. Specifically, senior reactor operators in the control room did not effectively coordinate manual main feedwater output adjustments in the control room or operator actions in the field in response to an apparent valve failure with the activities of non-licensed operators locally evaluating the equipment condition in the field.
