Summary of Corrective Actions -26.203(e)(2)
IAnalysis of Waiver Assessment Data:There were two (2) waivers requested and approved for required minimum shift coverage and extendedoutage work hours during the reporting period. Staffing was evaluated and determined to be adequate.
Oneindividual exceeded 16 work hrs in a 24 hr period, one exceeded 72 work hrs in a 7 day period.Analysis of Fatigue Assessment Data:Fifteen (15) fatigue assessments were conducted in accordance with procedure SY-AA-1 00-1011.
Therewere five (5) assessments for For Cause/FFD Drug and Alcohol events and ten (10) for Post-Event issues.Conclusions:
The conclusion for all but two fatigue assessments was that the involved personnel were not fatigued.
Twofatigue assessment resulted in terminated access for reasons other than fatigue.
The shift coverage issueswere isolated, but such occurrences are trended.Summary of Status of Corrective Actions:No corrective actions were performed as a result of the waivers or fatigue assessments.
