Title | LICENSEE-IDENTIFIED Violation |
Description | The licensee identified a finding of very low safety significance (Green) and an associated NCV of 10 CFR 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, for the failure of a control room operator to open the correct out-of-service reactor feed pump recirculation control valve during the conduct of performing a maintenance work order (WO 40105857). Specifically, on August 18, 2011, a control room operator incorrectly manipulated the in-service reactor feed pump recirculation control valve which was not in accordance with the clearance order required to depressurize a maintenance boundary. The operator immediately recognized the error, placed the valve into the closed position, and the issue was entered into the CAP as CR 01679118. No adverse plant response occurred as a result of the error. This failure to meet the requirements of WO 40105857 was a performance deficiency. The performance deficiency was more than minor because it was associated with the Initiating Events Cornerstone attribute of configuration control and its objective of limiting the likelihood of events that upset plant stability and challenge critical safety functions during shutdown and power operations. Specifically, had the operator not immediately recognized the error, opening of the incorrect reactor feed pump recirculation control valve would have likely resulted in a significant reactor level or temperature transient. The inspectors determined the finding could be evaluated using the SDP in accordance with IMC 0609.04, Phase 1 - Initial Screening and Characterization of Findings, Table 4a for the Initiating Events Cornerstone. Because the finding did not contribute to both the likelihood of a reactor trip and the likelihood that mitigation equipment or functions would not be available, the inspectors screened the finding as very low safety significance (Green). |
