APPENDIX A

NOTICE OF VIOLATION

Duquesne Light Company

Docket No. 50-412

Beaver Valley Power Station, Unit 2

License No. CPPR-105

As a result of the inspection conducted on May 23 - June 28, 1985, and in accor-

dance with the NRC Enforcement Policy (10 CFR 2, Appendix C), published in the

Federal Register on March 8, 1984 (49 FR 8583), the following violation was iden-

tified:

10 CFR 50, Appendix B, Criterion V requires that activities affecting

quality shall be prescribed by procedures and shall be accomplished in

accordance with these procedures.

Drawing 12241-BZ-19A-48-0C defines the dimensions, associated tolerances, and

other necessary details to properly install pipe support 2SWS-PSST 625.

Contrary to the above, and even though the installed support was " final in-

spected," the relative piping / support centerline dimension of pipe support

25WS-PSST 625 did not conform to that specified on Drawing 12241-BZ-19A-48-0C.

This is a Severity Level IV Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby re-

quired to submit to this office within thirty days of the date of the cover letter

to this Notice, a written statement or explanation in reply including: (1) the

corrective steps which have been taken and results achieved; (2) corrective steps

which will be taken to avoid further violations; and (3) the date when full com-

pliance is achieved. Where good cause is shown, consideration will be given to

extending your response time.
