ACCIDENT RADIATION MONITOR SPECIAL REPORT Descri tion of Event On September 22, 1990 at 0130, the Main Steam Line Radiation Monitor RM-01MS-3593SB was declared inoperable due to a communication failure and keyboard lockout. This monitor is identified in the Harris Technical Specifications Table 3.3.10 as Item 19.. The preplanned alternate method of monitoring the appropriate channels is to take hourly readings off the main steam/feedwater tunnel floor area monitor. This action was performed while the monitor was inoperable. The monitor was out of service for greater than 7 days; thus, a Special Report is required to be submitted per the Technical Specification 3[3.3.6.c.

Cause Corrective Action On September 22, 1990, Monitor ARM-01MS-3593SB was taken out of service due to a communication failure and keyboard lockout. The

logic power supply for the main steam line monitor was replaced .on September 25, 1990. The vendor representative suggested that the power supply be allowed to burn-in for several days. On October 3, 1990, MST-I0409 "Main Steam Line Monitor RM-01MS-3593-SB Calibration" was satisfactorily performed and the monitor was returned to service.
