On June 17, 2011, TVA provided verbal notification of this release to TDEC in conjunction with subsequent written reports dated June 20 and July 13, 2011 (above references).

10 CFR 50.72(b)(2)(xi) notification to the NRC (Event Notification 46967) was also made on June 17, 2011.In accordance with the WBN Unit 1 Environmental Protection Plan (EPP), Appendix B to Facility Operating License NPF-90, the NRC is to be provided a copy of the referenced letters. EPP Section 5.4.2 states, "Events reportable under this subsection that also require reports to other Federal, State or local agencies shall be reported in accordance with those reporting requirements in lieu of the requirements of this U.S. subsection.

Verbal notification was provided by telephone to the Chattanooga Environmental Field Office on June 17, 2011, at 09:48 EST. At no time has there been a threat to public drinking supplies, to human health, or the environment.

Furthermore, WBN has not observed fish distress, fish mortality, or abnormal bird migration since the day of discovery.

WATTS BAR NUCLEAR (WBN)INTAKE PUMPING STATION RELEASES ATTACHMENT Description of the Release and Determination of Cause: On June 16, 2011 at 13:09 EST, WBN personnel noticed a small steam of water releasing over a trough on the outside of the Intake Pumping Station (IPS) and entering the Intake Forebay.The water was determined to be raw water treated with sodium hypochorite and Flogard MS6237, a zinc chloride and phosphoric acid mixture. These chemicals are injected into plant systems per the WBN Biocide Corrosion Treatment Plan (BCTP). The chemical feeds were terminated at 13:20 EST (sodium hypochlorite) and 14:00 EST (Flogard MS6237). The water-releasefrorn the-trugh was-terminated at 14:30 EST. Samples in the Intake at the release were collected for total residual chlorine (TRC), pH, total phosphorus, and total zinc. TRC collected at 15:18 was initially 0.19 mg/L and decreased to less than <0.05 mg/L by 18:57 EST.The Daily Maximum Calculation for TRC was determined to be <0.09 mg/L. pH collected at 14:52 EST at the same location was 7.2. Total phosphorus and total zinc results are pending and will be provided in the June Discharge Monitoring Report (DMR).The cause of this event was determined to be an obstruction from vegetation in the trough that blocked the flow from the Essential Raw Cooling Water (ERCW) strainers, causing the water flow to release over the side of the trough. The trough is located approximately three quarters of the way up the south wall of the IPS and runs the entire length of the south side of the building.

It directs water from the ERCW strainer and traveling water screen backwash to the Yard Holding Pond which discharges to the Tennessee River via NPDES Ouffall 101.At 14:35 EST soon after the trough release was terminated, WBN personnel then noticed a release from a broken pipe on the same outerwall outside of the IPS with water entering the Intake Forebay. It was determined that this water was treated with Flogard MS6237. WBN personnel terminated the A and B Corrosion Control Chemical Dilution Water Supply to stop the flow from the broken pipe at 16:05 EST on June 16, 2011.At no time was there a threat to public drinking supplies, to human health, or the environment.

Furthermore, the locations of the releases are in close proximity of the plant intake and would likely be drawn through the IPS for plant use and discharged through permitted ouffalls.

TVA biologists surveyed by boat the Intake Forebay and downstream of the diffusers the evening of June 16, 2011. No fish distress, fish mortality, or abnormal bird migration was observed.Period of Release: The trough release was initially discovered on June 16, 2011 at 13:09 EST and was terminated at 14:30 EST on June 16, 2011. The broken pipe release was initially discovered on June 16, 2011 at 14:35 EST and was terminated at 16:05 EST on June 16, 2011. It is unknown when the releases began.itI A.

INTAKE PUMPING STATION RELEASES Page 2 Steps Being Taken to Reduce, Eliminate, and Prevent Recurrence:

WBN has documented the release in the TVA Corrective Action Program and is in the process of determining corrective actions to reduce and prevent recurrence of these releases.

Any updates will be be reported in the WBN June DMR.Immediate actions were to terminate sodium hypochlorite and Flogard MS6237 chemical injections.

Although a splash guard was added to the trough in 2010 to prevent a trough overflow, the obstruction from vegetation prevented the splash guard from working effectively.

The vegetation was removed at 19:00 EST on June 16, 2011. The trough capacity was then tested with maximum raw (untreated) water flow (e.g. three screen wash pumps in service) with no issues observed.

The broken pipe is currently scheduled for repair. After repair, validation of the repair will be conducted with raw water flow prior to the use of any treatment chemicals.

-DISCHARGE MONITORING REPORTS (DMR) FOR JUNE 2011 AND REVISED IMP 107 SHEETS Enclosed is the compliance paper copy of the DMR for the month of June 2011, the Hydrostatic Test Water DMR governed by NPDES Permit No. TNG670347, and revised DMR sheets for IMP 107.In accordance with NPDES permit number TNG670347, WBN discharged hydrostatic test water to the Tennessee River via Outfall 101. These discharges were in accordance with effluent limitations and monitoring requirements.

These monitoring results are included on a separate DMR sheet.During a records review, TVA noted a DMR software error which resulted in the duplicate samples not being averaged for the "daily maximum concentration".

In addition, the sample type was erroneously identified as composite instead of grab for several parameters.

WBN is submitting the revised IMP 107 DMR sheets for the past three years (3109, 9/09, 11/09, 5/10, 2/11, 4/11), to accurately reflect the "maximum" data, the sample frequencies, and the sample type. In addition, the software has been adjusted to reflect the appropriate number of decimals places for copper and iron to agree with the analytical results below the 1.0 rounding thresholds.

Finally, TVA is in the process of reviewing DMRs for any additional software errors.TVA submitted a written notification to the Chattanooga Environmental Field Office on June 20, 2011, describing the releases to the Intake Forebay that were discovered on June 16, 2011. The attached description provides the details of the event, including updates not available at the time of the initial written submittal.

E2-1 of 22 Ms. Dana Waits Page 2 July 13, 2011 At approximately 11:35 EST on June 29, 2011, WBN personnel discovered a small stream, of sanitary wastewater flowing across the floor from the basement lift station into the turbi¶e building station sump (TBSS). It was estimated that at most 100 gallons reached the nearly 58,000-gallon sump. The TBSS was immediately isolated to prevent mixing in the treatment ponds. Flow into the sewage injector tank was also Immediately isolated.

WBN began pumping the TBSS to Spring City's Publicly Owned Treatment Works (POTW) and temporary storage tanks to remove this wastewater from TBSS discharge.

The sanitary wastewater was effectively removed by feeding and bleeding greater than five times the volume of the TBSS to the POTW, an accepted engineering method for effectively removing contaminants.

This was confirmed through bacterial analysis prior to returning the TBSS to normal operation.

The cause was determined to be failure of the two sewage ejector pumps. At this time, one pump has been returned to service and the second is currently in the process of being repaired.

It should be noted that one pump is capable of meeting normal system demands. TVA believes that WBN's timely actions were appropriate In preventing environmental impact to the receiving stream. This issue is still under review by the site corrective action program. Based upon the results of the review next month corrective actions to prevent recurrence will be implemented.

WBN will update the Division as these actions are determined.

As an interim action, WBN has increased visual monitoring of the sewer ejector tank and flow into the sewage injector tank will remain isolated until a corrective action is implemented.
