Suggested Format for Providing Information Requested in Items 5 through 11 of NRC Form 313
- Appendix B is designed to be used for all types of applicants requesting a :Service provider license. All applicants should complete Tables B-1 through B--4, as applitJ:able.
After selecting the applicable items that need to be addressed, refer to the'. corresponding sections in the NUREG. It is necessary for applicants to provide the level :of detail required for the individual types of seivice(s) requested in the application. Note that p~oviding information for very limited licerises (e.g., leak-test service provider) requires less info_i-mation than would be required for a commercial nuclear laundry or a waste management licens~.
