(Mon) OPENED L2017-CRS- IN Boiling-Water Reactor Dryout Indications Occurring During Normal Operation 25 07/28/2017 0046 This IN is intended to inform addressees of recent operating experience related to numerous dryout indications which occurred over multiple cycles at the Leibstadt Nuclear Power Plant (NPP), a BWR/6 in Switzerland.

L2017-CRS- RIS Electronic Signature Use for Medical Licensee Internal Documents Required by NRC Regulation 24 08/24/2017 0048 This RIS is intended to supply information to assist medical use licensees in complying with the current NRC requirements related to the use of electronic signatures in internal licensee electronic documents and to clarify what constitutes a signature on electronic documents that medical use licensees are required to sign and maintain.

L2018-GEN- RIS Pre-Application Communication and Scheduling for Accident Tolerant Fuel Licensing Submittals 13 07/30/2018 0005 The staff has generated a list of questions regarding anticipated agency work in the area of accident tolerant fuels (ATF). This RIS will provide the venue for collecting that information.

L2018-GEN- IN Independent Spent Fuel Storage Installations Inspection Findings 10 10/30/2018 008 This IN is intended to inform addressees of recent NRC inspection findings that have identified non-compliance with NUREGs and applicable codes and standards.

L2018-GEN- IN Operating Experience Regarding Flow-Accelerated Corrosion 8 12/19/2018 0012 Flow-accelerated corrosion (FAC) is an industry issue that was first recognized to be of significant concern at Surry in 1986. There continue to be FAC failures attributed to problems with the FAC monitoring program and its software that have caused reactor trips.

L-2019- IN Recent Medical Events Associated with Yttrium-90 Microsphere Brachytherapy 7 01/15/2019 GEN-0005 The information notice alerts licensees to the potential for medical errors when the written directive is not properly completed or verified. The NRC expects that licensees will review the information for applicability to their facilities and consider actions, as appropriate, to avoid similar problems.

L-2019- IN - Counterfeit, Fraudulent, Suspect Item (CFSI) Training Offerings 7 1/17/2019 GEN-0002 This IN is a revision to the IN 2012-22. It updates a list of CFSI trainings available throughout the industry.

L-2019- IN - Strontium-82/Rubidium-82 Generator Breakthrough Events and Issues 1 08/2019 GEN-0010 The purpose of this IN is to inform addressees of incidents where excessive levels of Strontium-82 and Strontium-85 were unknowingly administered to patients during a cardiac imaging procedure because of failed Strontium-82/Rubidium-82 generators.

L-2018- IN Inadequate Evaluation of Temporary Alterations 2018 03/12/2019 GEN-0000 The U.S. Nuclear Regulatory Commission (NRC) is issuing this information notice (IN) to alert licensees about recent operating experience at nuclear power facilities where temporary alterations were installed that either negatively impacted the operability of affected Systems, Structures, and Components or without performing required 10 CFR 50.59 reviews.

L2017- RIS Reporting Requirements for Components Exported Under General License 10/22/2018 03/19/2019 CRS-0004 Provisions This RIS is intended to clarify the reporting requirements for nuclear facilities and equipment as required by 10 CFR 110.54 and the International Atomic Energy Agency for the Application of Safeguards in the United States.

L2018- IN Evaluations Supporting Changes to Plant Configuration 03/29/2018 03/18/2019 GEN-0000 This information notice is being issued to reiterate the need for comprehensive evaluations of configuration changes to structures, systems and components (SSC).

L2018- RIS Preparation and Scheduling of Operator Licensing Examinations 12/19/2018 04/18/2019 GEN-0011 This RIS is intended to inform addressees of the NRC staffs need for updated information on project site-specific operator licensing examination schedules and on the estimated number of applicants planning to take operator licensing examinations.

This information will help the NRC plan its resources more effectively.

This RIS update is intended to clarify the guidance contained in previously issued RIS 2005-29. RIS 2005-29 was initially issued to discuss concerns with demonstrating that certain events would not escalate into more serious events.

L2019- IN Effective Cyber Security Practices for Consideration to Protect Digital Assets 01/15/2019 08/14/2019 GEN-0000 of Byproduct Materials Licensees The purpose of this IN is to inform affected byproduct materials licensees of the results of an evaluation that was previously conducted by NRC staff of the potential need for cyber security requirements and of effective cyber security practices for licensees consideration. The INs enclosure will include the effective practices that provide the addressees information on contemporary cyber security issues related to the protection of digital assets, including those assets used to facilitate compliance with physical security requirements such as Title 10 of the Code of Federal Regulations (10 CFR)

Part 37, Physical Protection of Category 1 and Category 2 Quantities of Radioactive Material.

L-2019- IN Potential Over-Pressurization of High Specific-Activity Alpha-Emitting 01/15/2019 08/26/2019 GEN-0006 Radioactivity Sources A liquid source containing 1.5 mCi (55.5 MBq) of Americium 241 was stored in a sealed glass storage container for 12 years, which burst in August 2017, causing significant personnel exposure risk. The cause of this container failure is believed to be the generation of hydrogen gas through hydrolysis, which caused pressure buildup in the container, and which is likely to be a problem for similar high specific-activity alpha-emitting radionuclides in an aqueous solution.

L-2019- IN Patient Skin Contamination Events Associated with I-131 Mibg During 01/15/2019 08/26/2019 GEN-0004 Neuroblastoma Treatments The information notice alerts licensees to the potential patient contamination risks associated with therapeutic treatments with iodine-131 meta-iodobenzylguanidine (I-131 Mibg). The NRC expects that licensees will review the information for applicability to their facilities and consider actions, as appropriate, to avoid similar problems.
