On February 26, 1982 your office was informed that the 3A2 HPI normal makeup nozzle was found to have the thermal sleeve tack welds broken, and subsequent investigation revealed cracks in the safe end and upstream piping. This constitutes performance of components that requires remedial action to pre-vent operation in a manner less conservative than assumed in the accident analyses in the safety analysis report.
