Dear Mr. O'Reilly:
On February 26, 1982 your office was informed that the 3A2 HPI normal makeup nozzle was found to have the thermal sleeve tack welds broken, and subsequent investigation revealed cracks in the safe end and upstream piping. This constitutes performance of components that requires remedial action to pre-vent operation in a manner less conservative than assumed in the accident analyses in the safety analysis report. Reportable Occurrence Report R0-287/
82-04 providing details of this incident was to have been submitted to you by March 12, 1982, pursuant to Technical Specification 6.6.2.1.a(9). However, the investigation of this incident and the preparation of a report are incom-plete at this time.
It is expected that the investigation and report will be completed, reviewed, and submitted to you no later than April 9, 1982.
