| event text = On December 28, 2006, at about 2:30 am local time, while performing a radiography at the pumping facility in Kuparuk, Alaska, the radiography camera source failed to fully retract into the shielded position.  The radiographer contacted the Radiation Safety Officer who guided the radiographer in unscrewing the drive cables so that the source was able to be fully retracted.  No over-exposures or unplanned exposures occurred.

The camera is a INC Model IR-100.  The original source strength of the radiography camera Ir-192 source was 100 curies.  Kakivik Asset Management plans to contact the manufacturer of the camera about this occurrence.
