The ' inspection findings indicate that certain activ /c' ties appeared to violate

NRC requirements.

The third violation described in the enclosed Notice is similar to a violation

contained in the Notice sent to you by our letter dated June 13, 1986. 'Because

' "similar violations", as described in the NRC Enforcement Policy, are of

significant concern to the NRC, please give particular attention in your

response to the identification of the root cause of th$s problem and your

corrective action to prevent recurrence.

One ' additional violation described in paragraph 3 is under consideration for

escalated enforcement action. Accordingly it is not included in the enclosed

!

Notice and no response to this. violation is required at this time.

However,

please be advised that the number and characterization of this violation

described in- the enclosed Inspection Report may change as a result of further
