Gentlemen:

This refers to an Enforcement Conference conducted by Mr. C. E. Norelius and

May 12, 1989.- The conference concerned operation of your facility with the

while the High Pressure Coolant Injection (HPCI) system was declared inoperable.

We have concluded that the RCIC steam line flooding issue has resulted in a

Severity Level IV violation for failure to have adequate test controls. This

resulted in your failure to immediately begin a plant shutdown as required by

your Technical Specifications when RCIC and HPCI systems are inoperable. The

replacement of the limit switch assembly is viewed as untimely as evidenced by

previous unreliable operation. More prudent repair or replacement of this

switch was appropriate in this area. The prompt actions by your Shift Supervisor

to identify and correct the RCIC problem prevented the plant from exceeding the

twenty-four hour action statement requirement in your limiting condition for

operation (LC0).
