This letter is in response to unresolved items identified by the NRC Pump and Valve Operability Review Team (PVORT) on the draft safety evaluation report elven to lilinois Power on January 28, 1986.

Attached for your Staff's review are responses to four of the five open generic items and responses to the specific concerns of component 1E22-F010. The response for Open Generic Item 5 (Test vs. Analysis Issue) is pending the written response from the NRC of the results of a technical session held in Chicago, Illinois on January 28-29, 1986.

PVORT Generic Concern #7 - Valve Stem Problems Draft SSER Table 3.10.2.1 The NRC Audit-2 Concern states:

As'a result of the two audits, three valves have been found to have bent, twisted, or experienced stem failures.

IP should perform an investigation / evaluation to determine the total number of valves with this problem and the root cause.

The results of this effort are to be categorized and submitted.

Illinois Power's response is as follows:

The three valves identified during the PVORT audits with valve stem deficiencies are Limitorque Motor Operating Valves.

In order to efficiently respond to a variety of industry-related problems concerning Limitorque motor opera-tor valves, IP's Nuclear Station Engineering Department (NSED),

with the joint concurrence of Plant Staff and Qualify Assur-ance, developed the Limitorque Action Plan which is summarized in detail in Attachment 3A.

The status of the action item is as of December 23, 1985.

Many of the problem resolutions in this plan are being addressed during M07ATS testing. B provides a status for all safety-related valves which are included in the MOVATS testing program.

PVORT Generic Concern #8 - Operator Key Damage Draft SSER Table 3.10.2.1 The.NRC PVORT Audit-2 Concern states: Valves FC-015A, 015B, and 024 were found to have damage to the operator key valve shaft connection. This was attributed to the manual stops on the actuator not being properly adjusted. However, the root cause was never determined. IP should perform an evaluation / survey to determine if there is any other valve assembly (valve and actuator) that did or may have this improperly adjusted manual stop to ensure that there is no damage ,and operability is not impaired. Illinois Power's response is as follows: Condition Report 1-85-12-030 was initiated in order to address the above problem ona generic basis. The CR was then transmitted to NSED for an engineering evaluation to determine the Root Cause, Remedial Corrective Action, and Generic Corrective Action (see Attachment 4A - Memo DGT-85-434]. Letter DGT 85-434 From D. G. Tucker to J. F. Hampton, Dated 12/21/85. As requested in your reference letter, NSED mechanical section has performed an investigation of the condition described in CR 1-85-12-030 nnd recom-l mends the following engineering evaluation: Root Cause-The scope of this condition is limited to Posi-Seal Butterfly Valves. It orginated from operating the' valves before proper-ly adjusting their operator's full-closed limit switch. Adjust-ment to the limit switch is specified in GTP-55 to prevent the valve Disc from being driven into the seat (i.e. valve stop*) and causing a large force to be applied to the valve's stem and key. This condition occures when the valve disc hits the valve stop and the stem and key are the only components pre-venting the operator gear from continuing its rotation. ~ l-The valve stop is only used on Posi-Scal butterfly valves, which.is tho only type of valve this condition addresses. REMEDIAL CORRECTIVE ACTION-Valves IFC015A, B and IFC024A have been repaired by way of NCMR's 1-614 and-1-615. Prior damage to similar plant valves should be repaired by replacing those damaged keys and stems with Posi-Seal replacement parts. These valves will be identified in the inspection and testing l actions listed in the " Generic Corrective Action" section of this letter. GENERIC CORRECTIVE ACTION-For the remainder of those valves which still require GTP-55 testing, Start-Up should train their testing engineers to pay particular attention to sections 6.9 and 7.6 of the GTP-55 costing. Section 6.9 specifics that special pre-caution should be used to ensure that limit switches arc set to de-energi:o the relay logic to prevent " hammer" and that these limits be set to allow for any " coasting" of the valve af ter de encrci ation. Section 7.6 cautions the testing engineer that beforo energizing the operator, he should take valvo drift into account when cetting full open and full closed limit switches to prevent the valve from being driven hard against its backseat or into its seat. All Safety-Related Motor-Operated (M.O.) Valves are currently under goirg MOVATS testing which is scheduled to be completed by fuel load. If the limit switch is improperly set in relation to the valve stop, 'the"as-found" torque valve will be higher than the vendor maximum thrust value. This deficiency will generate an NCMR,' the disposition of the NQ!R will require the valve to be inspected for damage. If any damage is found it will be repaired in accordance with recommended procedure. To assure that the limit switch setting remains within a safe range, all M.0. valves will be inspected and adjusted in accordance with section 8.1.8 of CPS operating Procedure 8451.01 Rev. 4. During HFCS RST to RST Run, in support of PTP-DG-03, significant flow rate / discharge pressure changes occurred (flow decreased frem rated 5600 GPM to 3500 GPM in approxi=ately 40 seconds following HFCS iniciarion) and vibration / noise levels increased at the test return line orifice E22-D004.' Upon disassembly of orifice and spool, (1&lR B17187) a large piece of stainless steel was found in the piping downstream of the orifice place and the orifice place was da= aged. It appears additional foreign material may be located in upstream piping. Source of material is unknown at this ci=e. Re=edial and Generic Corrective Action: Initiated NCMR l-1957. Returnec to Startup for the following two reasons:

While troubleshooting per XTP-SP-02, it was discovered that in NSPS Division III when an analog trip module (ATM) was selected in either the monitor or monitor coccare mode, all the ATMs in the selected card file sequentially and intersittently tripped and reset. Further troubleshooting determined that PROM (s) has failed on the Division III Data Acquisition / Display Controller (DADC) board. Remedial and Generic Corrective Action: Initiated NCMR l-2041. 4rrAofmEMr 6A (hp [occle CR 1-85-11-003 .( Pago of N INVESTIGATION RESULTS Investigation has determined that prior to this Condition Report, excessive vibration was experienced in the HP piping as a result of flow orifice lE22-D004 being located too close to valve 1E22-F0ll. FECN 60336 permanently relocated this flow orifice, such that the vibration levels were restored to acceptable limits. Shortly thereafter, significant flow rate / discharge pressure changes and metallic noises were experienced in the HP piping during the HPCS RCIC Storcge Tank (RST) to RST run, in support of PTP-DG/D0-03. Flow orifice IE22-D004 was removed (MWR 17187) and one half of a valve stem retaining ring was found. The suspect source of the retaining ring were valves lE22-F010 or lE22-F011. Valve lE22-F010 was disassembled (MWR B13246) and the valve stem was discovered fractured. Valve IE22-F011 was disassembled (MWR B17188) and was found to be missing two halves of the retaining ring. The stem disc nut on IE22-F011 had loosened, apparently due to excessive vibration, allowing the retaining rings to be introduced into the system as loose parts. The other half or the retaining ring was finally retrieved, from inside the RCIC Storage Tank, using Tool Bo:: Skills.

t REMEDIAL CORRECTIVE ACTION The Engineering Evaluation for NCMR 01-1957 determined a new Orifice Plate lE22-D004 is to be fabricated and installed via MWR 17189.

The fractured valve stem on IE22-F010 is to be replaced and the valve restored to an acceptable configuration via FDDR LH1-5504 per the Engineering Evaluation of NCMR 01-1960. Valve IE22-F0ll will be restored to an acceptable configuration per FDDR LH1-5503 and reinstalled per MWR 17188. GENERIC CORRECTIVE ACTION The 1E22-F010 stem failure is being addressed generically via a 10CFR Part 21 Report (21-85-18). There is no Generic Corrective Action for valve 1E22-F011 because the vibration problem is considered a one time occurrence. NOTE Valves IE22-F010, lE22-F011 and IE22-F023 are the only active valves where the disc nut is attached by means of staking. l i
