DOCKET NOS. 50-445 AND 50-446 APPLICATION OF NON-QUALIFIED AGASTAT RELAYS SDAR: CP-86-40 (INTERIM REPORT)
Gentlemen:
On October 10, 1986, we notified you by our letter logged TXX-6017 of a deficiency involving the procurement and use of non-qualified Agastat relays in several Class IE applications which we deemed reportable under the provisions of 10CFR50.55(e). This is an interim report submitted to status corrective action implemented to date. Our most recent interim report was logged TXX-6313, dated March 2, 1987.
We have completed the identification of non-qualified relays affected by this deficiency and plan to implement the following corrective actions:
1.
All future Agastat 7000 series relays will be purchased with an "E" prefix for use in Class IE application, and will be segregated in the warehouse from other relays without an "E" prefix.
2.
7000 series Agastat relays without an "E" prefix, procured in the field with Q Code-C (commercial) for IE application and stored in the warehouse, are to be replaced with "E" prefixed relays.
Agastat 7000 series relays without the "E" prefix will be re-classified for use in non-1E applications.
3.
7000 series Agastat relays without an "E" prefix, procured in the field with Q Code-C and installed in safety related equipment, are to be replaced with "E" prefixed relays. The relays without an "E" prefix will be returned to the warehouse for use in non-1E applications.
4.
7000 series Agastat relays without an "E" prefix will be listed in
" Nuclear Operations Defective Items List" (N0DIL) to prevent the future use of the relay without an "E" prefix for IE application.

TXX-6475 May 29, 1987 Page 2 of 2 The' replacement of the non-qualified relays with qualified relays will ensure that the equipment installation conforms to the equipment qualification specifications. These actions will be accomplished in accordance with Nonconformance Report (NCR) 85-0004, Revision 3, and are scheduled for completion by September 18, 1987.
Also, during our review, we have determined that Westinghouse notified TV Electric at the time the equipment was shipped (as noted in the document package) that the equipment had been furnished with unqualified relays which would require subsequent replacement. The equipment was installed with the unqualified relays.
Programs are currently established to ensure similar conditions (i.e., installed equipment with unqualified components or equipment purchased in the future) are identified and dispositioned.
We will submit our next report no later than October 9,1987.
