DOCKET NOS. 50-445 AND 50-446 APPLICATION OF NON-QUALIFIED AGASTAT RELAYS SDAR: CP-86-40 (INTERIM REPORT)
Dear Mr. Johnson:
On May 28, 1986, we verbally notified your Mr. I. Barnes of ~a deficiency involving the procurement and use of non-qualified Agastat relays in several Class 1E applications. We have submitted interim reports logged TXX-4882 and TXX-4967, dated June 27, 1986, and August 18, 1986, respectively. We are reporting this issue under the provisions of 10CFR50.55(e) and the required information follows.
DESCRIPTION This issue resulted from a review and subsequent investigation of IE Information Notice No. 82-04, " Potential Deficiency of Certain Agastat E-7000 Series Time-Delay Relays".
Preliminary evaluation by site engineering noted a safety-related procurement specification wherein the subject relays were specified, but non-qualified relays were furnished by the equipment supplier. Additionally, some vendor originated design drawings did not specify a qualified relay as required.
SAFETY IMPLICATIONS In the event the deficiency had remained undetected, the indeterminate reliability of the timing relays could adversely affect the safety of plant operations.
CORRECTIVE ACTION Non-qualified relays will be identified by document review and inspections and will be scheduled for replacement. Vendor designed equipment drawings showing the application of Agastat 7000 series-timing relays will be reviewed and revised as necessary to require the use of pre-qualified ("E" prefixed) components for all future replacements.
Further, procurement procedures will be revised to preclude the purchase of non-qualified relays for CPSES applications.
