The following information was provided by … The following information was provided by the Louisiana Department of Environmental Quality (the Department) via email:</br>On March 19, 2023, Versa Integrity Group, Inc. notified the Department that there was a possible overexposure. The industrial radiography crew was working with a QSA 880 Delta (source projector) with 100 Ci of Ir-192 at Air Liquide, Geismar, LA. The camera serial number is D14952. The Ir-192 source serial number is 72308M.</br>The crew pocket ion chamber went off scale. </br>The industrial radiography source is secured and properly shielded within the QSA 880 Delta camera.</br>The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance:</br>The Department will be meeting with the licensee's radiation safety officer (RSO) and conducting an investigation tomorrow, March 20, 2023.an investigation tomorrow, March 20, 2023. +
