Title	Potential Overexposure
Event Description	The following information was provided by the Louisiana Department of Environmental Quality (the Department) via email: On March 19, 2023, Versa Integrity Group, Inc. notified the Department that there was a possible overexposure. The industrial radiography crew was working with a QSA 880 Delta [source projector] with 100 Ci of Ir-192 at Air Liquide, Geismar, LA. The camera serial number is D14952. The Ir-192 source serial number is 72308M. The crew pocket ion chamber went off scale. The industrial radiography source is secured and properly shielded within the QSA 880 Delta camera. The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance: The Department will be meeting with the licensee's radiation safety officer (RSO) and conducting an investigation tomorrow, March 20, 2023.
