The following information was received viaThe following information was received via email:</br> </br>On January 12, 2018, GL0704 Pacific Aurora, LLC (formerly Aurora Coop West) contacted (the Nebraska Department of Health and Human Services (NEDHHS)) regarding their overdue Annual Invoice and Current Inventory. The Environmental Manager (EM) noted that Aurora Coop West was bought by Pacific Ethanol on December 16, 2017 and has combined into one entity: Pacific Aurora, LLC. The EM indicated he was unable to locate several of the tritium exit signs on the inventory but would attempt location for one additional week. NEDHHS Office of Radiological Health received written notification on January 25, 2018 of one lost/missing tritium exit sign. The EM assumes the sign may have been moved during a renovation. Should the sign be located, the EM has indicated they will contact (NEDHHS).</br>Device Name: Radioluminescent Sign</br>Manufacturer: Safety Light Corp.</br>Model Number: SLX60</br>Serial Number: 418040</br>Radionuclide: H-3</br>Activity: 11.5 Ci.</br>NEDHHS Item Number: NE180002</br>THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL</br>Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
