The following is a summary of information … The following is a summary of information provided by Paragon Energy Solutions, LLC via email:</br>Tennessee Valley Authority (TVA) experienced binding in several actuators (Schneider Electric part number MA-418-0-0-4) used to operate dampers associated with emergency diesel generator room ventilation. Paragon identified that precipitate particles from the lubricating oil can collect in the space between the small motor rotor and the stator thus preventing rotation of the shaft connected to the damper actuating linkage. Paragon entered this issue into its corrective action program. Paragon conducted a detailed failure analysis which confirmed lubricating oil precipitate particles collecting in the gear train preventing rotation of the damper actuating shaft linkage. Additional investigation supports a conclusion that the extent of condition can be limited to the actuators supplied in 2017. Paragon suspects that the manufacturer may have inadvertently used oil already showing degradation during the manufacturing run. However, Paragon could not confirm with the vendor any potential deficiencies with the manufacturing processes or oil storage that would have contributed to this failure.
