A patient received an underdose of about 67 percent due to leakage at a connection point to the catheter.</br>The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance:</br>The leak appears to be caused from a crooked connection. Modifications are being made to the procedure to review the connections from different angles to assure alignment.</br>A Medical Event may indicate potential problems in a medical facility's use of radioactive materials.
