Title	Medical Event - Y-90 Underdose
Event Description	The following information was provided by the licensee via phone: A medical event involving Y-90 TheraSpheres occurred on 10/03/2024 at approximately 1000 EDT. A patient received an underdose of about 67 percent due to leakage at a connection point to the catheter. The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance: The leak appears to be caused from a crooked connection. Modifications are being made to the procedure to review the connections from different angles to assure alignment. A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
