On 12/31/15, at approximately 0630 hours ( … On 12/31/15, at approximately 0630 hours (EST), during shift turnover panel walk-downs, a licensed Unit 3 reactor operator identified that the High Pressure Coolant Injection (HPCI) flow controller output indicated a downscale condition. The controller was in automatic with the set point at 5000 gpm, which would typically indicate a controller output value of 100%. HPCI was not in operation and is a standby system. Operations promptly declared the HPCI system inoperable and entered Technical Specification Condition C for HPCI being inoperable. Other standby systems (Reactor Core Isolation Cooling and low pressure emergency core cooling systems) are OPERABLE. </br>HPCI is a single train system. Therefore, per NUREG-1022, this condition is being reported pursuant to 10 CFR 50.72(b)(3)(v)(D) as a condition that could have prevented the fulfillment of the safety function of a system required to mitigate the consequences of a design event. </br>This condition has been entered into the Corrective Action program (IR 2606215). Maintenance troubleshooting of the flow controller loop has identified a failed component and repair activities are in progress. </br> </br>The NRC Resident Inspector has been informed of this notification.or has been informed of this notification. +
