The inspectors reviewed a self-revealing n … The inspectors reviewed a self-revealing noncited violation of Technical Specification 5.4.1.a, Procedures, involving the failure to perform an adequate fill and vent of the component cooling water system which resulted in voiding of the system. The licensee entered the finding into their corrective action program and will develop corrective actions as part of Condition Report 33925. The inspectors determined that the failure to perform an adequate fill and vent of component cooling water that resulted in system voiding was a performance deficiency. The inspectors determined that this finding was more than minor because it is associated with the human performance attribute of the Mitigating Systems Cornerstone and it affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Inspection Manual Chapter 0609.04, Phase 1 Initial Screening and Characterization of Findings, the finding was determined to have very low safety significance (Green) because it did not result in a loss of system safety function, an actual loss of safety function of a single train for greater than its technical specification allowed outage time, or screen as potentially risk significant due to a seismic, flooding, or severe weather initiating event.
