The inspectors identified a finding of ver … The inspectors identified a finding of very low safety significance associated with an NCV of 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, because Constellation did not adequately implement scaffolding control requirements contained in MN-1-203, Scaffold Control. Specifically, Constellation did not perform engineering evaluations for scaffolding constructed within the minimum allowed distance of safety-related equipment. Constellation entered this issue into their CAP for resolution, took prompt actions to correct the scaffolds, and provided evaluations to assess the affect of the scaffold on the equipment. The evaluations determined that the scaffolds did not adversely affect the plant equipment. The inspectors determined that this finding is more than minor, because it is similar to example 4.a in Appendix E of IMC 0612 in that Constellation routinely did not perform evaluations for scaffolds constructed within the minimum allowed distance of safety related equipment. It is associated with the external factors and equipment performance attributes of the Mitigating Systems cornerstone and affects the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. The finding is of very low safety significance, because the finding is not a design or qualification deficiency, did not represent a loss of a safety function, and did not screen as potentially risk significant due to external events.
