A finding of very low safety significance … A finding of very low safety significance and an NCV of TS 3.8.1 was identified by the inspectors on February 23, 2012, due to the licensees failure to properly assess the continued operability of the D2 emergency diesel generator (EDG) following monthly surveillance testing. As a result, the D2 EDG was incorrectly declared operable with a known equipment deficiency. Corrective actions for this issue included declaring the D2 EDG inoperable, repairing the equipment deficiency, and requiring operability decisions to be reviewed by the shift manager. The inspectors determined that this finding was more than minor because it was associated with the human performance and equipment performance attributes of the Mitigating Systems Cornerstone. In addition, the performance deficiency impacted the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. The inspectors determined that this finding was of very low safety significance because each of the questions listed under the Mitigating Systems Cornerstone column of IMC 0609.04, Table 4A could be answered no.
