ENS 5546323 July 2021 04:00:0010 CFR 21.21(d)(3)(i), Failure to Comply or DefectPart 21 - Loose Connection on Egp/Etr Series RelaysThe following is a summary of information received from TE Connectivity via fax: During Management of Change review on 7/23/21, TE Connectivity discovered test load was unconnected to HI-POT tester for EGP/ETR series relays. TE's investigation found a connection remained loose after scheduled calibration testing on 12/30/2020. This report serves to document TE's evaluation and the corrective action taken on this issue: 1. A search of TE's database finds no occurrence of Dielectric Withstand Voltage (DWV) test failure at TE facility or similar failure with our customers in the field. 2. Many nuclear customers perform thorough inspections and testing prior to installing relays. 3. No customer history of EGP/ETR complaints of DWV failures found. 4. A series of corrective actions are now in place to guarantee full DWV testing to specification including: a mistake proof HI-POT test fixture developed and installed, and Hi-Pot test setup verification included as part of testing routine. There have been no reported field failures for the identified impacted produce. However, out of an abundance of caution TE will notify customers of the issue and the option to return any relays potentially related to this issue for retesting and recertification. ENS 528102 June 2017 04:00:0010 CFR 21.21(a)(2), Interim Report for Comply or Defect in ComponentPart 21 Notification - Potential Defect in Etr14 Class Te Connectivity Relays

TE Connectivity reported that one of their relays, TE Part Number 7-1423177-5, ETR141B3BNC2023004, failed at TVA Labs during normal continuously energized operation. An investigation conducted with TVA Labs identified that a mis-oriented capacitor may have caused the failure. Although the failure occurred with only one part number, all relays in the ETR14 family with date codes from 1604 to 1650 contain the suspect capacitor. A root cause analysis is underway by the vendor. ENS 524279 December 2016 05:00:0010 CFR 21.21(a)(2), Interim Report for Comply or Defect in ComponentPart 21 Notification - Defective Safety-Related Relays Supplied by Te ConnectivityThe following information is a summary of a letter of nonconformance submitted by TE Connectivity: Limerick Generating Station reported that four TE Connectivity supplied relays failed power labs analysis. The relays are TE part number 1-1431775, ETR14I3D004. The affected relays appear to be those in date code ranges 1604 and 1612. The preliminary evaluation revealed an incorrect orientation of one capacitor installed on the printed circuit board. The relays were purchased by Curtiss-Wright (8 relays), Limerick Generating Station (8 relays) and NextEra (2 relays). An investigation is ongoing.
