An investigation is currently ongoing to identify the cause of the elevated tritium levels. The tritium levels did not exceed any NRC regulations or reporting criteria. Tritium has not been detected in any other locations and is not expected to impact groundwater or exceed any limits in the Off Site Dose Calculation Manual (ODCM). This notification is being made solely as a four-hour, non-emergency notification for a Notification of Other Government Agency. This event is reportable in accordance with 10 CFR 50.72(b)(2)(xi). There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified. ENS 511982 July 2015 12:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertent Emergency Siren ActuationThis event is being reported in accordance with 10CFR50.72(b)(2)(xi). On July 2, 2015, at approximately 0830 EDT, an inadvertent actuation of the Perry Nuclear Power Plant's (PNPP) alert notification system occurred. Seventy-six of seventy-six sirens sounded for three minutes affecting the emergency planning zone in Ashtabula, Geauga, and Lake Counties. Following the actuation, county agencies received calls from members of the public. PNPP's capability to notify the public in an emergency was not affected. The siren actuation was not related to any condition or event at the PNPP. An investigation is in progress to determine the cause of the inadvertent actuation. Preliminarily, it appears that the wrong test was initiated from a county agency; an audible test was initiated instead of a silent test. At the time of the event, the plant was in Mode 1 at 100 percent rated thermal power. The public was informed of the inadvertent actuation by way of the Emergency Alert System (EAS). Additionally, social media was used to respond to social media inquiries. The NRC Resident Inspector has been notified. The licensee notified the Ohio Emergency Management Branch Chief, and the County Emergency Managers of Ashtabula, Geauga, and Lake Counties.

Unusual Event Declared Due to Toxic Gas Release

Release of toxic or flammable gas affecting the Protected Area boundary deemed detrimental to the safe operation of the plant. Emergency Action Level entered: MU-1. The leak is Trichloroethylene (TCE) gas used in the Off-Gas building. The Off-Gas building ground and basement levels were evacuated due to the leak. There is no safe-shutdown equipment located in the Off-Gas building. The licensee is working to isolate the leak. The licensee informed the NRC Resident Inspector. The licensee notified the State of Ohio and the local counties. Notified DHS SWO, FEMA Ops Center, NICC Watch Officer, DOE Ops Center, USDA Ops Center, HHS Ops Center, and Nuclear SSA via email.

Unusual Event has been terminated on 4/5/2014 at 0059 EDT. The trichloroethylene leak has been stopped. Access has been restored to all normally accessible areas. Unit 1 remains in Mode 1 at 100% power. The licensee notified the NRC Resident Inspector and the Local and State emergency agencies. Notified the IRD MOC (Gott), R3DO (Passehl), and NRR EO (McGinty). Notified DHS SWO, DOE Ops Center, FEMA Ops Center, HHS Ops Center, NICC Watch Officer, USDA OPS Center, EPA EOC, FDA EOC, and Nuclear SSA via email.

ENS 4974621 January 2014 06:40:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Tritium Discovered in GroundwaterThis notification is being made pursuant to (10 CFR) 50.72(b)(2)(xi), notification of other government agency. Notification to other government agency, State of Ohio, was made at 0140 (EST) on 1/21/14. At 1310 on 1/20/2014, a leak was identified on a feed water Venturi. In response to the water leak, samples were taken to check for the spread of tritium. A positive result for tritium was identified in the under drain system in the Auxiliary Building which requires communications as part of the NEI ground water protection initiative. The positive sample results were obtained at 2330 on 1/20/14. Actions are in progress stop the leak (perform leak injection). The EPA limit for groundwater is 20,000 pCi/l. The samples taken by the licensee indicated 46,000 pCi/l. The licensee has notified the NRC Resident Inspector and will notify local counties.

Transport of Potentially Contaminated Worker and Subsequent Fatality

On February 19, 2013, at approximately 1303 EST, the control room was notified that a supplemental worker (i.e., a contract individual) had fallen and was injured. The worker was in a contaminated area. Due to the individual's condition, the individual was not surveyed by a Health Physics technician prior to being transported in their anti-contamination clothing. The individual was transported by ambulance accompanied by Health Physics personnel to the local hospital for medical treatment (i.e., TriPoint Medical Center). Subsequently, the worker was declared deceased at the hospital. This notification is being made in accordance with 10 CFR 50.72(b)(3)(xii) and 50.72(b)(2)(xi). Additionally, OSHA was notified pursuant to the requirements of 29 CFR 1904.39. The Lake County Coroner was also notified. Subsequent surveys found no contamination on the worker, hospital, medical personnel, or ambulance. No press release is planned. The NRC Resident Inspector has been notified.

On May 7, 2012, at approximately 1233 hours (EDT), an inadvertent actuation of the Perry Nuclear Power Plant's alert notification system occurred. Twenty of the seventy-six total sirens sounded for three minutes affecting Ashtabula, Geauga, and Lake Counties. Following the actuation the county agencies received calls from members of the public. A successful quiet test of the sirens had been conducted earlier in the day (at approximately 0830 hours (EDT)). At this time, all sirens are functioning correctly. The siren actuation was not related to any condition or event at the Perry Nuclear Power Plant. The actuation signal originated from the Lake County Emergency Operations Center while thunderstorms were passing through the area. Additional investigation is in progress to determine the cause of the inadvertent actuation. At the time of the event, the plant was in Mode 1 at 100 percent rated thermal power. Lake County officials plant to issue a press release. The NRC Resident Inspector has been notified.
ENS 479016 May 2012 13:35:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Made for Npdes NoncomplianceThis event is being reported in accordance with 10 CFR 50.72(b)(2)(xi). On May 6, 2012, during daily chlorination activities, it was identified that the National Pollutant Discharge Elimination System (NPDES) permit limit for Total Residual Chlorine was exceeded between approximately 0935 hours (EDT) and 0947 hours (EDT) when the noncompliance was corrected. The maximum measured value was 0.29 mg/L, which exceeded the NPDES Maximum Concentration Limit of 0.2 mg/L. On May 6, 2012, at approximately 1930 hours (EDT), a 'Noncompliance Notification for Exceedance of a Daily Maximum Discharge Limit' was made to the Ohio Environmental Protection Agency. The cause of the NPDES permit noncompliance is under investigation. Chlorination evolutions have been suspended pending investigation results. At the time of the event, the plant was in Mode 1 at 100 percent rated thermal power. This event is also being reported in accordance with the plant's operating license, appendix B, Environmental Protection Plan, which states, in part, that any occurrence of an unusual or important event that indicates or could result in environmental impact causally related to plant operation shall be recorded and reported to the NRC within 24 hours followed by a written report. Offsite Notification Due to Fuel Oil SpillThis event is being reported in accordance with 10CFR50.72(b)(2)(xi). On October 21, 2011, at approximately 1723 hours EDT, notification of a fuel oil spill was made to the US Environmental Protection Agency (EPA), National Response Center. At the time of the event, the plant was in Mode 1 at 100 percent rated thermal power. The fuel oil spill was caused by a leak in an underground pipe outside the protected area, but inside the owner controlled area. The spill is estimated between 1000 to 1500 gallons and is contained onsite. However, a certified wetlands specialist was contacted and determined the area met the criteria for wetlands designation, which in turn made the event reportable. Clean Harbors Incorporated is assisting with the onsite clean-up and remediation. Additionally, the Ohio EPA: State Emergency Response Commission, Perry Township Fire Department, Lake County Emergency Planning Committee, and the U.S. Coast Guard were notified in accordance with plant procedures. This event is also being reported in accordance with the plant's Operating License, Appendix B, Environmental Protection Plan, which states, in part, that any occurrence of an unusual or important event that indicates or could result in significant environmental impact causally related to plant operation shall be recorded and reported to the NRC within 24 hours followed by a written report. The licensee notified the NRC Resident Inspector.
ENS 4680329 April 2011 15:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Fatality of a Contract EmployeeOn April 29, 2011, at approximately 1100 hours, a contract employee was found unresponsive in a vehicle, off the road, inside the Perry Nuclear Power Plant owner controlled area. At 1120 hours, it was confirmed by the Perry Fire Department that the individual was deceased. Notification has been made to the Lake County Sheriffs department and the Lake County Coroner. The death does not appear to be work related or the result of an accident. A preliminary diagnosis by the coroner is the individual suffered a heart attack. The Lake County Coroner is conducting a routine investigation into the death. A press release is not planned at this time. The individuals name has not yet been released pending notification of next-of-kin. A notification to OSHA per 29CFR1904.39 will be made for this event. The plant is currently in MODE 5 (Refuel) for a refueling outage. This event is being reported in accordance with 10 CFR 50.72(b)(2)(xi) as an event or situation, related to the health and safety of the public or on-site personnel, for which notification to other government agencies has been or will be made. Such an event may include an on-site fatality. The NRC Senior Resident Inspector has been notified.

Offsite Notification Due to Sodium Hypochlorite Leak from Underground PipingOn October 28, 2010, at 1740 hours, the plant entered the Off-Normal Instruction for spills and unauthorized discharges. At 1751 hours, notification of a sodium hypochlorite spill was made to the U.S. Environmental Protection Agency (EPA), National Response Center. At the time of the event, the plant was in Mode 1 at 100% power. The sodium hypochlorite spill is believed to be the result of a leak in an underground piping supply line to the Emergency Service Water pump house. The leak is estimated to be approximately 130 gallons of a 12.5% Sodium Hypochlorite (Bleach) solution (80 gallons in a 24 hour period is the reportable quantity). The associated chlorination systems have been isolated, and current storage tank level is stable, indicating no more leakage in progress. Additionally, the Ohio EPA: State Emergency Response Commission, Perry Township Fire Department, Lake County Emergency Planning Committee, and the U.S. Coast Guard were notified in accordance with plant procedures. This event is also being reported in accordance with the Operating License, Appendix B, Environment Protection Plan, which states in part, 'Any occurrence of an unusual or important event that indicates or could result in significant environmental impact causally related to plant operation shall be recorded and reported to the NRC within 24 hours followed by a written report ..' ENS 4579828 March 2010 04:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleasePress Release Due to Lube Oil Fire Affecting Reactor Feed Pump

At 1818 , the control room was notified of a lube oil fire on Reactor Feed Pump Turbine B. The fire brigade was toned out, and the Perry Township Fire Department was notified for assistance. Reactor Feed Pump Turbine B was manually tripped to allow removing the turbine lube oil system from service. The Motor Feed Pump started as expected, and the Reactor Recirculation system lowered reactor power as designed. Currently the reactor is at 68% reactor power following rod line adjustments after the transient was complete. The fire was reported to be out by the fire brigade leader at 2122. Damage assessment is currently in progress. Two fire brigade members were transported by ambulance to Tri-Point Medical Center with signs of heat exhaustion. Both individuals were verified to be free of contamination prior to transport. A news release will be made due to local media interest for this event. The NRC Resident Inspector has been notified.

The purpose of this call is to update Event Number 45798. In the notification made on 3/28/10, it was stated that a news release would be made due to local media interest for this event. After further review, it was determined that discussions with the media adequately addressed the interest and that a new release will not be made. The NRC Resident Inspector has been notified. ENS 454239 October 2009 19:44:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to a Spill of Hydraulic FluidOn October 9, 2009, at approximately 1544 hours EDT, notification of an oil spill was made to the U.S. Environmental Protection Agency (EPA), National Response Center. At the time of the event, the plant was in Mode 1 at 100% power. The oil spill was the result of delivery activities related to a backhoe that was delivered to the Perry Nuclear Power Plant in the owner controlled area. The spill was initially estimated to be approximately 3 to 5 gallons of a red colored oil resembling hydraulic fluid. With current rainy weather conditions, the resultant sheen covered an area of approximately 20 to 25 feet by 100 feet of gravel and asphalt. The spill drained into two storm drains. The backhoe was moved inside of a building with a concrete floor. Absorbent material and oil selective soak mulch were placed on the area of the spill. Clean Harbors Incorporated (CHI) was contacted for clean up assistance. CHI representatives are providing assistance for clean-up remediation. Additionally, the Ohio EPA: State Emergency Response Commission, Perry Township Fire Department, Lake County Emergency Planning committee, and the U.S. Coast Guard were notified in accordance with plant procedures. This event is also being reported in accordance with the Operating License, Appendix B, Environmental Protection Plan, which states in part, 'Any occurrence of an unusual or important event that indicates or could result in significant environmental impact causally related to plant operation shall be recorded and reported to the NRC within 24 hours followed by written report. Specifically, unanticipated or emergency discharge of waste water or chemical substances.' The licensee has notified the NRC Resident Inspector.
ENS 4411131 March 2008 19:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to Ohio Epa Concerning Non-Compliance with Hazardous Waste Removal Requirements

At 1550 hours, a self identification call was made to the State of Ohio Environmental Protection Agency (EPA), offices in Columbus, Ohio to inform them of the recent discovery of non-compliance issues relating to the accumulation, storage, and shipment (i.e., 90 days to ship) of hazardous waste (reference 40 CFR 261, 'Identification and Listing of Hazardous Waste'). Guidance was requested from the state EPA for remediation and reporting of the condition. The Perry Nuclear Power Plant personnel were advised by the Ohio EPA to properly identify, package and ship the waste as soon as possible. The waste consists of three drums of floor grinding waste generated during the resurfacing of the Auxiliary Building floor in 2005, and approximately 100 bags of miscellaneous trash generated during disassembly of plant equipment in 2007. This issue was discovered during activities to prepare the waste for shipment off site. The waste will be shipped per the direction of the Ohio EPA and an out-of-cycle hazardous waste annual report will be made to the Ohio EPA. This report is being made in accordance with 10 CFR 50.72(b)(2)(xi) as an event or situation related to the protection of the environment for which a notification to another government agency has been made. The resident inspector has been notified. The hazardous material contains low level rad waste.

On March 31, 2008, in accordance with 10 CFR 50.72(b)(2)(xi), notification was made for an event or situation related to the protection of the environment for which a notification to another government agency was made. This report was made when a self-identification call was made to the State of Ohio Environmental Protection Agency (EPA) offices in Columbus, Ohio, for the discovery of potential non-compliance issues with 40 CFR 261 requirements for the accumulation, storage, and shipment of hazardous waste. Based on further investigation using a Toxicity Characterization Leachate Procedure analysis of the waste, it was determined that the waste was, in fact, non-hazardous and could be shipped for off-site disposal. The requirements of 40 CFR 261 had been and continue to be met. The licensee has notified the NRC Resident Inspector and the State of Ohio. Notified R3DO ( Pelke).

Offsite Notification of Unknown Discharge and 24 Hour Report Per Appendix B of Operating License

(The information below concerns a) September 19, 2007 NPDES Reportable Occurrence verbiage/Notification to the Ohio Environmental Protection Agency. This notification was made on September 20, 2007. Time of discharge is unknown. Cloudy water was discovered in 'Major Stream' diversion (also referred to as 'Red Mill Run') at approximately 1525 hours on September 19, 2007.

The amount of discharge is unknown. At the time of discovery, approximately 10 to 15 gallons of water, per minute, were observed discharging from the storm drain. On September 20, 2007, the observed discharge from the storm drain was approximately 1 to 2 gallons of water per minute. The pH of the stream was 11.2 standard units (SU) at 1650 hours on September 19, 2007 with white powdery deposition on the stream bed. At 1000 hours on September 20, 2007, a sample taken for pH indicated 8.48 SU. A number of small stressed fish were observed at time of discovery, along with approximately 5 to 10 small dead fish. No frogs, crayfish, or other local aquatic life forms were observed to have been negatively impacted. Initial analysis of the sediment from the stream bed indicated the main constituents were inorganic calcium & oxygen (possible materials include but not limited to calcium oxide, calcium carbonate, calcium hydroxide).

The source of the high pH and white sediment is unknown at this time and an investigation is in progress.

The affected area (has been inspected) and recommendations for clean-up remediation (provided). The recommendations are currently being evaluated.

This event is being reported in accordance with Appendix B to the Operating License (The Environmental Protection Plan) which requires reporting of any occurrence of an unusual or important event that indicates or could result in significant environmental impact causally related to plant operation (An example includes 'fish kills'). This event is also being reported in accordance with 10CFR50.72(b)(2)(xi). 'Any event or situation, related to the health and safety of the public or on-site personnel, or protection of the environment, for which a news release is planned or notification to other government agencies has been or will be made. Such an event may include an on-site facility or inadvertent release of radioactively contaminated materials'. The NRC Resident Inspector and applicable State authorities have been notified.

Potentially Contaminated Person with a Back Injury Transported to Lake East Hospital

At 2005, an injury occurred to a worker when the individual fell and injured his back. At 2122, the worker was attended by local Emergency Medical Personnel who transported the individual to our local Hospital. Due to his back injury his back will not be able to be completely monitored until arrival to the hospital. The individual is being classified as potentially contaminated. Radiation (Protection) personnel are accompanying the victim on the ambulance and to the local hospital emergency room to aid in radiological assessment. Due to our procedures we will be making notifications to the State Of Ohio and the local county Emergency Management Agencies of the incident. Due to the offsite notifications a 4 hour non-emergency call is required. The individual was a contractor onsite for outage support. The licensee notified the NRC Resident Inspector.

The contractor individual was found in the Containment Drywell 630 foot elevation. He fell off of scaffolding approximately 4-5 feet to the grating. When the medical personnel arrived at the scene they found him unconscious. He regained (consciousness) when medical personnel arrived at the scene. At 2218 (the licensee received a) report from Radiation Protection personnel that the individual was not contaminated. Notified R3DO (L. Kozak).

The injured person was found not to have contamination on him but it was reported to Perry Nuclear Power plant that fixed contamination was found on his Protective Clothing (PC), Safety Belt and Tool Belt. Fixed contamination on the PC direct contact reading was between 100 to 150 cpm. Fixed direct on contact contamination on the Safety Belt & the Toll Belt was between 100 to 500 cpm. No contamination found in the ambulance or in the Hospital. NRC Resident Inspector was left a message of this information by the licensee. NRC R3DO (L. Kozak) notified.

ENS 426938 July 2006 13:18:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification - Exceeded Limit for Suspended Solids in Discharge

On 6/29/06 at 1630 Neutralization Basin A was pumped to Lake Erie. On 7/7/06 the analysis of suspended solids for this discharge was completed with results outside of the daily limit. This was follow-up information regarding event number 42693 ('Offsite Notification - exceeded limit for suspended solids in discharge'). On 7/10/06 final data analysis for NPDES Monthly Report was obtained and the 6/29/06 exceedance resulted in an exceedance in the monthly average of total suspended solids (TSS). Results for monthly average suspended solids were 44.2 ppm with a limit of 30 ppm. State of Ohio EPA was notified of this non-compliance at 0925 on 7/11/06. ENS 4267730 June 2006 19:18:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseNon-Credible Security Threat Made Against PlantNotified by Security Shift Supervisor of a report by the Ashtabula County Sheriff that a 92 year old individual has been checked into the Ashtabula Clinic at 14:57 EDT, claiming to have an explosive device at his Kingsville home and at the Perry Power Plant. This is being considered as a non-credible security threat, based on his never having access to the site, ONI-P56-2 is not applicable, based on this report and assessment. Law enforcement officers are obtaining a warrant to access his home to investigate. The FBI and Lake County Sheriff's offices have been notified by the Security Shift Supervisor. This is an off site report from the Perry Plant to a government agency about the status of the Perry Plant. The Lake County Sheriff's office completed their search of individual's residence and found no bombs or related material. The licensee notified the NRC Resident Inspector.
ENS 424765 April 2006 16:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to State and Local Officials Concerning Tritium Detection in Drain SystemThis notification is being made in accordance with 10CFR50.72(b)(2)(xi), i.e., notification of a government agency. FENOC provided a courtesy notification to state and local officials based on the detection of very low levels of tritium in the plant underdrain system. During routine quarterly sampling of the Perry plant's underdrain system on March 28, 2006, very low levels of tritium were detected in an underdrain system manhole. The plant's underdrain system directs water under the plant to the Emergency Service Water Pumphouse, where it is monitored and discharged. Samples taken off-site show no indications of detectable tritium. No reportable limits have been exceeded. The NRC Resident Inspector has been notified.Service water

Alert Declared Due to a Fire in a Ventilation Fan

At 1506 EST, Operators heard a loud noise and discovered that a bearing in the Motor Control Switchgear in the Miscellaneous Equipment Area Return Fan "B" had failed. The Plant Fire Brigade responded and extinguished the fire. Offsite assistance was requested and did respond. No injuries were reported and all personnel have been accounted for. A reflash watch has been posted. The licensee will remain in the Alert awaiting completion of the ventilation transfer to the "A" train. The licensee notified state/local agencies and the NRC Resident Inspector.

At 1740 EST, the licensee terminated the Alert based on the fire being out and the plant stable. The licensee plans to issue a press release. ENS 4232910 February 2006 18:45:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite NotificationAt 1345, a notification was made to the Ohio EPA. This notification was based on 1) Any unanticipated bypass which exceeds any effluent limitation in the NPDES Permit and 2) Any discharge of water to the storm drains that is not covered by the following excerpt from the permit. The event itself was the pumping of rain water that had sulfuric acid (pH approximately 2.6) in it to a storm drain. Approximately 10 gallons of this mixture was pumped to the drain. The 10 gallons pumped contained approximately 1 milliliter of sulfuric acid. Sample taken at the storm drain out fall indicated normal pH value of 8.03. The licensee notified the NRC Resident Inspector.
