ENS 5204527 June 2016 01:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to an Oil Spill

The United States Coast Guard reported an oil sheen in the vicinity of the station's circulating water system effluent. Investigation by station personnel has not determined the source. The circulating water pumps were secured to mitigate the potential source. The United States Coast Guard response Center, and New York State Department of Environmental Conservation have been notified. The licensee notified the NRC Resident Inspector. Notified DOE, EPA, USDA, HHS, FEMA.

ENS 5172210 February 2016 07:47:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertent Actuation of One Emergency SirenThe purpose of this report is to provide a telephone notification under 10 CFR 50.72(b)(2)(xi) to notify the NRC of the inadvertent actuation of one Oswego County notification siren at approximately 0247 (EST) on 02/10/2016. The James A. FitzPatrick Control Room was notified by Oswego County 911 at 0359 on 02/10/2016 of the inadvertent actuation. It is unknown at this time as to why the inadvertent alarm actuated. Siren repair personnel (ANS Services) have been dispatched to isolate the siren and begin repair work. The siren has since been silenced. Alternate notification of the public in the area is through Hyper Reach. The Oswego County Emergency Management Office issued a News Release identifying the inadvertent actuation of the emergency siren. The licensee will be notifying the NRC Resident Inspector.
ENS 5169429 January 2016 13:12:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification - Sewage Treatment Plant Discharge Limit Exceeded

On January 29, 2016 James A. FitzPatrick Nuclear Power Plant (JAF) received notification from the site Sewage Treatment Plant (STP) operators that the January 6, 2016 monthly settleable solids result for the STP was 0.2 ml/L/hr (milliliter/liter/hour). This value exceeds the State Pollutant Discharge Elimination System (SPDES) permit limit of 0.1 ml/L/hr (daily maximum). The STP operators conduct daily process control tests at the STP and did not identify any system upset issues around the January 6, 2016 sample date, or any time since, that would be symptomatic of the slightly elevated settleable solids result. The JAF environmental engineer concluded that a notification to the New York State Department of Environmental Conservation (NYSDEC) was not required for this event; however, a courtesy notification for permit noncompliance was made. The NYSDEC has been notified. Pursuant to 10 CFR 50.72(b)(2)(xi), this condition is being reported as an event or situation for which notification to a government agency has been made. The NRC resident has been notified. JAF is currently at 0 percent power in Mode 2 following a forced outage resultant of events on January 23, 2016.

Based on further review of the NRC reporting guidance relative to this criteria, JAF has concluded that this condition is below the reporting threshold outlined in NUREG-1022 Revision 3. NUREG-1022 states the following (page 54), 'Licensees generally do not have to report media and government interactions unless they are related to the radiological health and safety of the public or onsite personnel, or protection of the environment.' The condition originally reported in ENS 51694 is considered a minor deviation in sewage process limits, and has no impact on the radiological health and safety of the public or onsite personnel, or protection of the environment. Therefore, JAF is retracting ENS 51694. The NRC Resident Inspector has been notified. Notified R1DO (Bickett).

ENS 5038921 August 2014 12:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertent Actuation of One Emergency Siren

The purpose of this report is to provide a telephone notification under 10 CFR 50.72(b)(2)(xi) to notify the NRC of the inadvertent actuation of one Oswego County emergency notification siren at approximately 0850 (EDT) on 08/21/14. Thunderstorms in the area are believed to have caused a lightning strike and the spurious activation. Siren repair personnel have been dispatched to isolate the siren and begin repair work. The siren has since been silenced. Alternate notification of the public in the area is through Hyperreach. The Oswego County Emergency Management Office issued a news release identifying the inadvertent actuation of the emergency siren. The NRC Resident Inspector has been notified.

This release came from the cafeteria kitchen walk-in cooler. The licensee notified the NRC Resident Inspector.

Oswego County was performing routine weekly testing and siren #17 was inadvertently actuated for approximately 2 minutes. The Oswego County Emergency Management Office issued a News Release identifying the inadvertent actuation of the emergency siren. The NRC Resident Inspector has been notified.

The purpose of this report is to provide a telephone notification under 10CFR50.72(b)(2)(xi) to notify the NRC of the inadvertent actuation of Oswego County emergency notification siren number 34 at approximately 17:40 EDT on 7/13/12. Personnel have been dispatched to the siren location and it has been deactivated pending further investigation. Backup measures are in place to notify residents if an actual emergency were to occur while the siren is out of service. The NRC Senior Resident Inspector has been notified. Oswego County Emergency Management Office has issued a press release as follows: EMERGENCY SIREN SOUNDS INADVERTENTLY Fulton - One of the emergency notification sirens surrounding the nuclear power plants in Scriba inadvertently sounded at about 5:40 p.m. today, July 13, due to a malfunction. There is NO emergency at the three nuclear power plants in Oswego County. (The) Director of the Oswego County Emergency Management Office, said siren repair personnel from Constellation Energy and Entergy responded to the site near the intersection of Middle Road and Route 104 in Scriba as soon as they were notified the siren was sounding. The cause for the sounding is under investigation. The other sirens in the system are functioning properly. While the siren system is being repaired, backup measures are in place to notify residents if an actual emergency occurs at one of the plants. The siren is one of 37 located within a 10-mile radius of the nuclear power plants in Scriba. A system of sirens and tone-alert weather radios is in place to alert residents in the event of an emergency. In the event of an emergency at one of the plants that would require protective actions for the public, the sirens would be sounded for three minutes to alert residents of the area to turn their AM/FM radios or televisions to local Emergency Alert System (EAS) stations for further information and instructions. EAS stations are listed in the Emergency Public Information Calendar mailed to residents of the Emergency Planning Zone surrounding the plants and in local telephone books. ENS 471285 August 2011 23:32:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Emergency Notification Siren ActuationThe purpose of this report is to provide a telephone notification under 10CFR50.72(b)(2)(xi) to notify the NRC of the inadvertent actuation of one Oswego County Emergency notification siren at approximately 1932 EDT on 8/5/2011. Initial notification to the James A Fitzpatrick control room of the siren activation was via the RECS (Radiation Emergency Communication System) Notification System. Power was removed to the affected siren by off-site repair personnel at 1948 EDT. The affected siren provides coverage to Oswego County. The sirens are utility owned and shared with the Nine Mile Point site. In the event the sirens are needed, the county has its Hyper-Reach (911 call back system) on standby. This issue has been entered into the station corrective action program. The NRC Sr. Resident Inspector has been notified. The licensee notified the State of New York and Oswego County. ENS 466596 March 2011 04:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseSpurious Siren ActiviationThe purpose of this report is to provide a telephone notification under 10 CFR 50.72(b)(2)(xi) to notify the NRC of the inadvertent actuation of one Oswego County emergency notification siren at approximately 2315 EST on 3/5/11. Initial notification to the JFA (James A. Fitzpatrick) Control Room of the siren activation was via on-site security personnel and verified with Oswego County 911 Center. The faulted siren was alarming intermittently and repair personnel were dispatched to correct the problem. At 0107 EST, power to the affected siren was de-energized by off-site repair personnel. Sirens affected provide coverage to Oswego County. The sirens are utility owned and shared with the Nine Mile Point site. In the event the sirens are needed the county has it's Hyper-Reach (911 call back system) on standby. The NRC Senior Resident Inspector has been notified.
ENS 4628127 September 2010 04:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification - Loss of One Emergency Response SirenOne of the 37 Prompt Notification System sirens surrounding the James A. FitzPatrick (JAF) /Nine Mile Point (NMP) sites spuriously activated at 0848 for a period of 3-5 minutes. This has been confirmed by siren report back system software and siren repair personnel. A member of the public called to report the activation to the Oswego County 911 Dispatch Center. The Dispatch Center notified Oswego County Emergency Management Office. County personnel took action to cancel the activation using the remote activation control system, notified the licensee emergency preparedness personnel, and dispatched a siren repair crew. Siren repair personnel have isolated the siren and are troubleshooting to determine the cause. No press releases are currently planned by either Oswego County or the licensees. The licensee notified the NRC Resident Inspector.

OFFSITE NOTIFICATION FOR ELEVATED TRITIUM LEVELS

On 12/23/09, station management was notified that a sample taken from (James A. Fitzpatrick) JAFs west storm drain was positive for tritium. The sample results were confirmed at 984 pCi/L. The sensitivity of the analysis is 800 pCi/L. The Offsite Dose Calculation Manual (ODCM) lower limit of detection (LLD) requirement for tritium is 3,000 pCi/L with a reporting level of 30,000 pCi/L. Comparing the confirmed activity level in the sample with the reporting criteria, there is little environmental impact, as the sample is more than an order of magnitude below the reporting criteria.

Although no regulatory limit was exceeded these results were evaluated against the NEI 07-07 guidance for voluntary informal communication of state/local authorities. Since the storm drains communicate with native soil and Lake Ontario, it has been determined that the voluntary informal communication criteria applies. That determination was made at 1515 (EST) on 12/28/09.

Since the state and local agencies will be notified of the slightly positive indication in the storm drain sample, reporting criterion 10 CFR 50.72(b)(2)(xi) was reviewed and based on the public sensitivity to tritium issues, it was determined that a notification to the NRC should be made.

The site has developed an action plan to address locating the source of the tritium (including) potential remediation actions, inclusion in the annual Radiological Environmental Monitoring Program (REMP) report, and evaluation for inclusion as an NRC IN 80-10 pathway. In the interim, both the reactor building perimeter sump and west storm drain have been placed on an increased sampling frequency. The storm drain will be sampled weekly and the reactor building perimeter sump will be sampled daily.

Background: On 11/03/09 station management was notified that a sample from the reactor building perimeter sump was positive for tritium at 1474 pCi/L. The reactor building perimeter sump was placed on increased monitoring frequency due to the positive indication. Since the sump only communicates with the environment, through the west storm drain, and there was no positive indication of tritium in the west storm drain or in the ground water monitoring wells, it was determined that no 'voluntary informal communication' criteria applied, at that time.

The licensee has notified the NRC Resident Inspector.

The licensee will notify the state, local, and other government agencies.

ENS 4367228 September 2007 17:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseEmergency Siren Activated Inadvertently During a Thunderstorm

A single siren in the Nine Mile/FitzPatrick EPZ was inadvertently activated coincident with a thunderstorm and lightning strikes in the area of the siren. Oswego Co (New York) Emergency management officials are issuing a press release regarding the storm related siren activation. Attempts to silence the siren were not effective using the systems remote activation system further indicating that lightning affected the siren.

A work crew has been dispatched to the siren to investigate and has silenced the siren. Notification is being provided to the NRC in regards to the potential for media inquiry. The licensee informed the NRC Resident Inspector.

ENS 4249613 April 2006 14:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertant Actuation of Emergency Notification SirensThe purpose of this report is to provide a telephone notification under 10CFR50.72(b)(2)(xi) to notify the NRC of the inadvertent actuation of the Oswego County emergency notification sirens at approximately 1030 on 4/13/06. Oswego County is participating in a radiological exercise involving a simulated emergency at the James A. FitzPatrick Nuclear Power Plant. It was during the simulated exercise that the sirens were inadvertently actuated. The Oswego County Emergency Management Office issued a News Release identifying the inadvertent actuation of the emergency sirens. The NRC Sr. Resident Inspector has been notified.

Primary Containment Inoperable Due to Small Crack in Torus

The licensee has determined that the small weep hole crack in the torus (x in shape) is in a position which requires the plant to initiate a normal shutdown to cold shutdown conditions. The crack is located about 5 feet below the waterline and just below the HPCI exhaust pipe discharge. Due to the location of this crack the licensee's engineering staff showed via analysis that it would be prudent to restrict the number of thermal cycles the crack receives. The plant is in a 1 hour Technical Specification LCO shutdown action statement. At 19:50 the licensee commenced the Technical Specification shutdown to cold shutdown conditions. As an additional item, one of the four Emergency Diesel Generators was deemed inoperable at the time of the event, and continues to be in a LCO action statement. The licensee made notifications to the State and local agencies, and will notify the NRC Resident Inspector.

With the plant operating at 100% on June 27, 2005 during RCIC Torus Suction piping inspection per ST-24H, inspectors discovered a TORUS leak in the vicinity of a TORUS Support between Bays A and P. The leak was quantified as slight weepage with streaking and a small puddle below leak. This was captured in the plant corrective action program on CR-JAF-2005-02593. Subsequent non destructive examination determined that the leakage was from a small through wall crack. The condition was discussed with the NRC Staff at that time. Initial evaluation at that time determined that there was reasonable expectation of operability, an Operational Decision Making Instruction (ODMI) was prepared and implemented to monitor the leakage while further engineering evaluation was conducted. On June 30,2005 further engineering analysis determined that operability of the primary containment was not assured and at 19:29 on June 30, 2005 the Shift Manager declared Primary Containment Inoperable and entered Technical Specification LCO 3.6.1.1 Condition A, Primary Containment Inoperable, and declared an Unusual Event under EAL 9.1.2. Preparations were made for a reactor shutdown and the shutdown was commenced at 20:00. At the time of the event the A Train of EDGs was out of service for a planned maintenance LCO. At this time the NRC Resident has been informed. The A Train EDG is not available nor operable if needed. The R1DO (White) and NRR EO (Carpenter) have been notified.

An Unusual Event was terminated at 1716 EDT due to achieving cold shutdown. R1DO (John White), NRR EO (Cynthia Carpenter), IRD Manager (M. Leach), FEMA (Todd Kuzia) and DHS (Craig) notified. NRC Resident Inspector, State and Local officials were notified by the licensee.
