SUBJECT:

SHUTDOWN GREATER THAN 48 HOURS Excessive shaft seal leakage was identified late on June 29, 1989 while performing inservice testing (IST) of a low pressure safety injection (LPSI) pump. The system had already been out of service about 2-1/2 days for preventive maintenance, with the 72-hour action statement to expire at about 8:00 a.m. (all times PDT) on June 30. The licensee was unable to reduce the seal leakage, and initiated a plant shutdown at 6:30 a.m. on June 30.

An Unusual Event (UE) was declared at 6:32 a.m.

as prescribed by plant procedures.

The UE was terminated at 7:45 a.m. when power was reduced below 60%, a point below which. Combustion Engineering stated that fuel damage will not be encountered by any analyzed accident.

During the shutdown the licensee performed a walkdown to verify that primary plant instrumentation (PPI) transmitters properly conformed to equipment qualification (EQ) requirements. A total of 26 instruments were found which did not have provision for draining of moisture from the instrument. These will be corrected before unit restart. Repair of the LPSI pump is also nearly complete, with the unit expected to enter Mode 3 late on July 3.

An entry into the Unit 2 containment identified EQ discrepancies similar to those in Unit 3. Justification for continued operation (JCO) has been provided for all but three instruments in PPS Channel 3, which has been declared inoperable.

The resident inspector is following the licensee's outage activities. Region V and NRR are also reviewing the licensee's JCO for Unit 2.
