At the beginning of March the DAEC was four days into Refuel Outage XIll, having removed the Main Generator from service February 24. The reactor had been placed in cold shutdown, disassembly of the reactor had been completed, and disassembly of the Main Generator and a Low Pressure Turbine was in progress.
Removal of fuel from the reactor to the spent fuel pool began March 3 and was completed at 4:00 p.m. March 6.
Leakrate testing of all eight main steam isolation valves was completed March 2 with only one valve (the "A" inboard) requiring maintenance and re-testing. The circulating water pit was refilled March 3 following minor repairs to piping joints and cracked and spalled concrete.
At 0940 March 7, five Primary Containment isolation Valves closed, Secondary Containment isolated, and Standby Gas Treatment initiated when a lead momentarily lost contact during the installation of ajumper while performing a surveillance test on the 24 Volt DC Battery System. At the time the plant was in cold shutdown for refueling, no fuel was in the reactor vessel, and neither primary nor secondary containment was required. The next day a similar event occurred during the installation of a Temporary Modification to support maintenance.
All automatic actions occurred as designed and the isolations were reset. Actions to prevent such inadvertent automatic actuations will be reviewed with appropriate plant organizations. There was no effect on plant operation or safety. LER #95-04.
Chemical decontamination of the Reactor Recirculation and Reactor Water Cleanup System piping was completed March 15 and the reactor vessel level lowered March 16. Chemical decontamination then reduced recirculation discharge piping dose rates by a factcr of two to three times the pre-decontamination levels.
Approximately 90 curies were removed during this decontamination compared to 15 curies in past chemical decontaminations.
Reassembly of the Low Pressure Turbine began March 15. Flood up of the refueling cavity was completed March 16 and set-up began for installing the ultrasonic testing scanner used to examine the core shroud welds and invessel inspection sites. Return of"A-side" safety systems was completed March 19.
During the In-Vessel-Visual-Inspection (IVVI), a shallow six-inch indication on the Core Spray sparger piping support bracket cladding was found. A diver successfully removed the surface indication with some light erinding. The IVV1 was completed March 24.
The core shroud ultrasonic weld testing finished early on March 29. DAEC is the first plant to successfully perform the scanning of the lower two welds on the core shroud, and the entire core shroud inspection was accomplished in less than twelve days. All the welds were found to be in a sound condition.
Core reload commenced just after midnight March 29 and was completed April 3.
