PURPOSE AND SCOPE 1.1 Purpose This procedure describes the turnover, isolation, and access controls for survey units prior to performing final status survey (FSS) at the Fort Calhoun Station (FCS) Decommissioning Project. It also provides the administrative process to evaluate, approve, and document decommissioning or maintenance related activities in survey units during and following FSS activities.

1.2 Scope Planned decommissioning actions must be completed and assessed for isolation and control measures before FSS activities can begin in a designated survey unit.

This procedure describes the protocols used to verify and document that a survey unit is in suitable physical condition for FSS implementation, that proper isolation and control measures are in place to limit the potential for cross-contamination from other decommissioning activities, and that the the final configuration of the survey unit is maintained.

This procedure applies to all personnel performing turnover and control activities in support of FSS.

GENERAL 3.1 Definitions 3.1.1 Access Control: Application of programs and policies to ensure that survey units are not cross-contaminated by personnel (or decommissioning activities) which may require access to survey units under FSS isolation and control.

3.1.2 Final Status Survey: Measurements and sampling to describe the radiological conditions of a site, following completion of decontamination activities (if any) in preparation for release.

Final Status Survey Isolation and Control Measures FCSD-RA-LT-303 Revision 1 Proprietary Information Page 5 of 17 3.1.3 Isolation and Control (I&C): The physical and the administrative means and methods by which a survey unit is identified, bounded, posted, and communicated to project personnel after turnover is completed.

3.1.4 Surveillance

Activities (which can include field inspections, self-assessments, radiological surveys, documentation reviews, and survey unit inspections) to verify that I&C measures plus access control successfully protects the physical and radiological condition for survey units where FSS has been performed.

3.1.5 Turnover

Acknowledgement of all cognizant project personnel that a system, structure, or open land survey unit meets all the physical and radiological conditions necessary for the License Termination/Final Status Survey (LT/FSS) Group to perform FSS.

3.1.6 Walk-Down

The inspection of a survey unit which assesses and confirms the readiness of that survey unit for turnover.

3.2 Abbreviations FSS final status survey ISOCS In Situ Object Counting System I&C isolation and control LT/FSS license termination/final status survey RA radiological assessment RASS remedial action support survey 3.3 Responsibilities 3.3.1 LT/FSS Manager Provides technical oversight and administration of this procedure.

Assesses and approves any decommissioning or maintenance related activities in survey units during and following FSS activities.

3.3.2 Director FCS Decommissioning Project (or designee)

Ensures decommissioning or remediation activities in the survey unit are completed.

Ensures housekeeping is performed, any tools or equipment not required to support FSS activities are removed, and any additional measures necessary to limit the spread of contamination from adjacent areas are implemented.

3.3.3 LT/FSS Specialist Reviews existing radiological survey data (e.g., characterizations, surveillances, radiological assessments [RA], remedial action support

Provides direction as to the content and extent of any RA or RASS performed in support of survey unit turnover.

Determines the frequency and type of surveillances that will be performed in survey units following the performance of FSS and prior to license termination.

3.3.4 LT/FSS Supervisor Maintains cognizance of areas nearing decommissioning completion.

Ensures decommissioning or remediation activities in survey units deemed ready for FSS are complete, and initiates the turnover process.

Assures site safety concerns, issues, and hazards are thoroughly addressed in the execution of this procedure.

Verifies that survey units to be surveyed for FSS are isolated, posted, and controlled.

Reviews RA or RASS performed in support of survey unit turnover.

Ensures access control measures are maintained for survey units where FSS activities are active or completed.

Maintains a log of personnel entering posted survey units under FSS isolation and control.

3.3.5 Survey Mapping/CAD/GIS Specialist Prepares drawings and other graphics (as necessary) to be included with any RA or RASS performed for survey unit turnover.

Generates surveillance drawings as directed by the LT/FSS Supervisor or LT/FSS Specialist.

3.3.6 LT/FSS Technician Establishes isolation and control prior to and during FSS activities.

Performs surveys and sampling as directed by supervision.

3.3.7 All Project Personnel Read and comply with FSS postings.

3.4 Precautions, Limitations, and Prerequisites 3.4.1 Precautions

When a survey unit is designated for turnover to support FSS, no radioactive material may be introduced, or decommissioning activities may take place in that survey unit, without an LT/FSS Supervisor assessment and LT/FSS Manager approval.

B. Postings and signs used to denote radiologically controlled areas shall not be used to denote FSS units. An example of a posting used to denote an FSS unit is provided in Attachment 5.1.

C. The LT/FSS Manager must be notified if the physical configuration or radiological conditions in a survey unit change where FSS activities are active or have been completed, regardless if the change was due to remediation or for other reasons.

3.4.2 Limitations A. I&C measures and access control shall be maintained for each survey unit until there is no risk of cross-contamination from decommissioning activities, or the survey units I&C is superseded by consolidation into a larger group of survey units (e.g., expanding posted barriers).

B. All forms described in this procedure may be generated electronically. If electronic forms are used, the physical layout of the form may be modified provided the intent described in this procedure is not changed.

C. FSS I&C boundary controls and postings shall be removed from FSS units just prior to backfill. The backfilled survey unit has a very low risk of cross-contamination for future FSS evaluation.

D. Limited ground disturbances (e.g., regrading of areas) in areas under I&C may occur prior to site release as long as the proper surveillance protocols are followed. Final site grading will not be performed until after license termination.

3.4.3 Prerequisites A. Characterization, historical surveys, RA, or RASS results should provide evidence that residual radioactivity is unlikely to exceed the unrestricted release criterion.

B. Decommissioning activities having the potential to cross-contaminate the survey unit must be completed prior to the initiation of this procedure.

C. Tools and equipment not required to perform FSS must be removed from the survey unit prior to turnover. Exceptions may be made by the LT/FSS Manager.

D. Housekeeping and cleanup of the survey unit must be completed prior to the initiation of turnover activities.

Posting materials used for posting of survey units should be easily recognizable as pertaining to FSS. Examples of these materials are green and white protective coverings such as tarps, herculite, plastic, labels, ropes, and signs.

3.5 Records 3.5.1 Records generated by this procedure are listed below and shall be processed per references 2.1.1, 2.1.2, and 2.1.3.

REQUIREMENTS AND GUIDANCE 4.1 Initiate Area Turnover and Walk-Down i

NOTE i

Upon completion of commodity removal, decontamination, or demolition activities in a designated FSS unit, the LT/FSS Group shall be notified that the decommissioning activities in the survey unit is complete and is ready for FSS. This communication may occur formally (e.g., by schedule, memorandum, or through project controls) or informally (e.g., by verbal communication).

4.1.1 When a survey unit is deemed ready for FSS, then the LT/FSS Supervisor will initiate FCSD-RA-LT-303-F-01, Area Turnover and Control Checklist, for the survey unit and document the following information:

Final Status Survey Isolation and Control Measures FCSD-RA-LT-303 Revision 1 Proprietary Information Page 9 of 17 4.1.3 The LT/FSS Supervisor will schedule and coordinate a walk-down of the survey unit to include all cognizant personnel (e.g., LT/FSS Supervision, LT/FSS Specialist, ES Director FCS Decommissioning Project, FCS Plant Manager, or designees) and denote the scheduled date, time, and attendees for the walk-down on FCSD-RA-LT-303-F-01 in the appropriate space.

Decommissioning and remediation activities are complete when all commodities and wastes have been removed from a survey unit, and all remaining materials have been decontaminated to levels less than the unrestricted release criteria or applicable action level.

4.1.4 During the walk-down, the attendees will assess the readiness of the survey unit for turnover, paying attention to the following items as delineated on FCSD-RA-LT-303-F-01 in the Walk-down for Completion of Decommissioning and Remediation Activities section:

A. All decommissioning activities in the survey unit are complete, including removal, as necessary, of items (e.g., subsurface systems, electrical components) that could interfere with FSS.

B. All surfaces to be surveyed are prepared for FSS as necessary (e.g.,

interferences removed, paint removed, penetrations exposed).

C. All decommissioning activities in areas adjacent to the survey unit to be isolated or that could otherwise affect it are either complete or are deemed not to have any reasonable potential to spread radioactive material to the survey unit. Any and all additional measures necessary to limit the spread of contamination from adjacent areas undergoing decommissioning (e.g.,

plug vent louvers, cover floor grating) have been implemented.

D. All tools and equipment not needed for FSS are removed.

E. Housekeeping has occurred and is satisfactory for the performance of FSS.

F. All support apparatus (e.g., ladders, scaffold) and equipment used for FSS activities does not pose the potential for introducing or spreading plant-related radioactivity in the survey unit.

G. All transit paths to or through the survey unit, except those required to support FSS, are eliminated or re-routed where practical.

4.1.5 If unsatisfactory conditions are identified, then number and document those conditions in the Items for Resolution spaces of FCSD-RA-LT-303-F-01.

As items for resolution are corrected, the LT/FSS Supervisor will indicate acceptance of the resolution by checking off, dating, and initialing the appropriate spaces in the Items for Resolution section of FCSD-RA-LT-303-F-01.

4.1.6 Indicate the presence of any Observed or Potential Hazards by checking the appropriate boxes on FCSD-RA-LT-303-F-01.

4.1.7 Document under Hazards Mitigation Measures any measures that have been taken or will be taken to mitigate the observed or potential hazards.

Include the names of persons responsible for mitigating observed or potential hazards.

4.1.8 Indicate the presence of any Conditions or Physical Constraints That May Affect Survey Performance by checking the appropriate boxes on FCSD-RA-LT-303-F-01.

4.1.9 Document under Resolution any corrective actions for Step 4.1.8 and include the names of persons responsible for those corrective actions.

4.1.10 Indicate necessary activities in the Survey Unit Preparation for Survey by checking the appropriate boxes on FCSD-RA-LT-303-F-01.

4.1.11 Document under Preparation Details any necessary activities for Step 4.1.10 and include the names of persons responsible for those activities.

4.1.12 Upon satisfactory completion of all activities identified in Steps 4.1.4 through 4.1.11, the LT/FSS Supervisor will obtain the Director FCS Decommissioning Project (or designee) signature, and then the LT/FSS Supervisor will countersign affirming the survey unit is acceptable for performing FSS.

A. These signatures acknowledge turnover of the survey unit from D&D to the LT/FSS Group and are entered on the appropriate spaces of FCSD-RA-LT-303-F-01.

4.1.13 The LT/FSS Supervisor may direct and oversee the implementation of temporary work control postings, materials, and barriers to maintain the physical and radiological integrity of the survey unit during turnover surveys prior to formally establishing I&C.

4.2 Area Turnover Survey and Posting for I&C 4.2.1 An LT/FSS Specialist will review characterization, RA, or RASS data for the survey unit to determine if satisfactory data is available to perform FSS design.

During the walk-down, the LT/FSS Specialist should emphasize and document any indications that would potentially impact the classification of Class 2 or Class 3 survey units.

The LT/FSS Specialist may waive the performance of a turnover survey only if the results from previous surveys (e.g., characterization, RA, RASS) indicate a low probability that the survey unit contains residual contamination above the applicable action level or DCGL, and the survey unit conforms to the variability criteria of FCSD-RA-LT-302, Final Status Survey Sample Plan Development (Ref. 2.1.4) and was not remediated prior to the performance of FSS.

4.2.2 The LT/FSS Specialist will indicate whether a turnover survey is required by checking the Yes or No box in the Survey for Area Turnover section on FCSD-RA-LT-303-F-01.

A. If yes, the LT/FSS Specialist shall design a turnover RA or RASS in accordance with FCSD-RA-LT-306, Radiological Assessments and Remedial Action Support Surveys (Ref. 2.1.5).

1. The same type of instrumentation and instrument sensitivity that will be used for FSS should be used to assess a survey unit for turnover.
2. If the existing radiological survey data is adequate and sufficient for FSS design, then any survey performed for turnover should consist of judgmental scans only.
3. If the existing radiological survey data is inadequate or insufficient for FSS design, the variability in the existing data does not conform to the criteria of reference 2.1.4, or if the survey unit was remediated prior to the performance of FSS, then the survey design from the RA or RASS will be sufficient to ensure that remediation in the survey unit was sufficient to meet the unrestricted release criteria and adequate survey data variability is available to perform survey design for FSS.
4. The LT/FSS Specialist will denote the RA or RASS sample plan number on FCSD-RA-LT-303-F-01.

4.2.3 The LT/FSS Specialist will sign the Survey for Area Turnover section of FCSD-RA-LT-303-F-01.

4.2.4 As directed, the LT/FSS Technician will perform a turnover survey of the survey unit using the instructions provided in the RA or RASS sample plan.

Final Status Survey Isolation and Control Measures FCSD-RA-LT-303 Revision 1 Proprietary Information Page 12 of 17 4.2.5 After completion of the turnover survey, the LT/FSS Technician will sign the Survey for Area Turnover section of FCSD-RA-LT-303-F-01.

4.2.6 After review of the turnover survey data, the LT/FSS Supervisor will sign the Survey for Area Turnover section of FCSD-RA-LT-303-F-01, attach the results of the turnover survey (including survey maps, field notes, and data), and notify the LT/FSS Specialist of turnover survey completion.

4.2.7 The LT/FSS Specialist will review the turnover survey results.

A. If the survey unit is acceptable for FSS, then the LT/FSS Specialist will denote approval by completing and signing the Comments and Observations section of FCSD-RA-LT-303-F-01 and notify the LT/FSS Supervisor the survey unit can be posted for FSS I&C.

1. If turnover survey data indicates the survey unit will require reclassification, then the LT/FSS Specialist may accept the survey unit for FSS with LT/FSS Manager review and approval. All supporting documentation for this review and approval must be referenced in the Comments and Observations section of FCSD-RA-LT-303-F-01.

B. If the survey unit is not acceptable for FSS, then cease the turnover process and inform the Director FCS Decommissioning Project (or designee) that the survey unit requires additional remediation. Document the unacceptable conditions in the Comments and Observations section of FCSD-RA-LT-303-F-01 and secure it into the appropriate FSS unit package folder. Initiate a new FCSD-RA-LT-303-F-01 after the survey unit is remediated, and restart this procedure for the survey unit.

4.2.8 The LT/FSS Supervisor will direct implementation of I&C for the survey unit.

FSS units should have I&C barriers and postings placed at all entrances. Postings should be highly visible to notify personnel of access restrictions and requirements.

A. Post all sides and possible access locations of FSS units using signs such as those illustrated in Attachment 5.1.

NOTE i

Posting the survey unit is the minimum physical requirement for I&C. Other controls include physical restrictions such as stanchions and barricades; use of personnel frisking stations, sticky pads, and protective clothing to prevent the introduction of plant-related radioactivity into the survey unit; personnel training, briefing, and site wide notices to disseminate information and provide instruction; or any combination of the above. These measures should be considered commensurate with the potential for introducing plant-related radioactivity into the survey unit.

4.2.9 The LT/FSS Supervisor will document the controls implemented for the survey unit in the Posting and Controls section of FCSD-RA-LT-303-F-01.

4.2.10 The LT/FSS Supervisor will sign the completed Postings and Controls section of FCSD-RA-LT-303-F-01 and file the completed form in the appropriate FSS unit package folder.

4.3 Access Control 4.3.1 Entry of personnel or equipment into survey units under FSS I&C is allowed only under the restrictions of access control as delineated in this procedure.

A. Access control may be implemented through the use of specific notification to specified persons or groups, for specified operations or inspections, on a case by case basis, as approved by the LT/FSS Manager.

1. Personnel who require access to the switchyard for routine operations are not required to notify the LT/FSS Group of switchyard entries for these routine operations.

B. These specific notifications will be documented and approved by the LT/FSS Manager, and the approval documents (original or copies) will be maintained in the affected survey units FSS package folder.

4.3.2 Any personnel entry or work activities shall receive authorization from the LT/FSS Supervisor prior to entering any survey units under FSS I&C.

4.3.3 The LT/FSS Supervisor will review and approve all access requests to FSS I&C survey units utilizing FCSD-RA-LT-303-F-02, FSS Area Access Request, Authorization, and Sign-In Sheet.

A. Document the requesting work groups contact person, the activities to be performed, and the expected duration with start and stop dates on FCSD-RA-LT-303-F-02.

Assess and determine if there is any potential impact from the introduction of plant-related radioactivity into the survey unit and which actions, if any, must be taken to prevent cross-contamination.

C. Summarize the conclusions of the assessment and document the access controls required in the Access Controls Required section of FCSD-RA-LT-303-F-02.

1. If access cannot be approved for the planned activities, then annotate Denied in the Access Controls Required section of FCSD-RA-LT-303-F-02.

D. All personnel, tools and equipment shall be surveyed for the presence of residual radioactive material or radioactive contamination prior to entering survey units under FSS I&C. The minimum survey requirement should be a hand and foot frisk.

1. Exceptions to this survey requirement are allowed under the specific notifications in Step 4.3.1.A of this procedure.

4.3.4 All personnel entering and exiting survey units under FSS I&C will annotate on FCSD-RA-LT-303-F-02 for every entry and every exit which is controlled by the work activities described on the form, and persons may only enter and exit during the duration for which the work activities are approved.

4.3.5 FCSD-RA-LT-303-F-02 will be maintained in the appropriate FSS unit package folder and will serve as a log for tracking all approved activities and access to the survey unit.

4.4 Post-FSS Routine and Special Surveillances 4.4.1 The LT/FSS Supervisor shall generate and maintain a list of survey units that have completed FSS and require surveillance, and document entries on FCSD-RA-LT-303-F-04, FSS I&C Surveillance Matrix.

Surveillances are not required in FSS units that have been backfilled or in piping (embedded or buried) that has been isolated or grouted in place.

4.4.2 Routine surveillances will be performed in survey units following FSS to monitor for indications of cross-contamination and verification of postings and access control measures.

A. Perform routine surveillances of each FSS unit on a semi-annual basis.

Surveillances may be performed as directed by supervision at any time that an FSS unit may be assessed to be physically or radiologically impacted by work activities within or adjacent to the FSS unit (e.g.,

transporting a radioactive material package though an FSS unit).

C. Routine surveillances may cease when there is minimal risk of recontamination from decommissioning or the survey unit has been released from the license. Special surveillances may still be performed as necessary.

4.4.3 Perform surveillances as follows:

A. Review the authorizations and reasons for entry into the survey unit since the performance of FSS or the last surveillance.

B. Perform a walk-down of the survey unit, checking:

1. that proper postings, barriers, system covers, and system seals are in place and intact;
2. for evidence of disturbance, loss of isolation and control, or unauthorized introduction of or placement of radioactive materials; and
3. that signs, postings, labels, and barriers are legible, accurate, and obvious to the casual observer.

C. Document the survey area, survey unit, survey unit type, classification, surveillance reason, surveillance activities, comments, observations, and whether the results are satisfactory or unsatisfactory on FCSD-RA-LT-303-F-03, FSS I&C Surveillance Summary.

D. If the results from steps A and B are not satisfactory, then perform and document a judgmental scan of the survey unit, focusing on access and egress points and any areas of disturbance or concern using the same type of instrumentation, scan speed, and alarm set-points as used in the FSS.

E. Document any verification surveys on a survey map of the survey unit.

4.4.4 Forward the completed FCSD-RA-LT-303-F-03 with any supporting documents, surveys, and maps to the LT/FSS Specialist for review and LT/FSS Manager for approval.

4.4.5 Update FCSD-RA-LT-303-F-04, FSS I&C Surveillance Matrix, annotating the completed surveillance date, and enter the next scheduled surveillance date.
