2.1 Workers shall perform Individual Hazard Analysis (IHA) for this task, which includes continual self-analysis of work scope, hazards and controls (reference Savannah River Site General Employee Basic Hazard Control Handbook).

2.2 A Time Out shall be initiated, FLM shall be notified and/or the work package returned to planning for problems or conditions encountered that are not anticipated or covered under the AHA or associated permits, or the boundaries/scope of work change as the job progresses. Workers shall perform Individual Hazard Analysis (IHA) for this task, which includes continual self-analysis of work scope, hazards and controls (reference Savannah River Site General Employee Basic Hazard Control Handbook).

2.3 Warning barricades are required for work areas to control unauthorized access to the work area, identify hazards associated with work, and prevent personnel contact with falling objects and moving parts of equipment.

Note: Achievement of equipment grouting may include the use of any of the following approved applications:

• Bucket OR

• Hand Pump As long as it is approved by CDE or LWGS, there is not a specified application for any one piece of equipment.

Note: If a piece of equipment/pipe or something is encountered that is unidentified in the work package, STOP WORK, and contact CDE for resolution.

4.365 Construction: Mix Equipment Grout; the quantity of grout to fill the section of pipe is approximately; 1.2 Gallon. /

4.367 Construction: ENSURE a Spill Kit has been staged in Valve House, and in proximity to any/all work locations where hose or funnel will be manipulated (disconnected/reconnected) on a regular basis. In the event of grout material spill:

CONTACT the control room, STABILIZE the work area and NOTIFY the LWGS of the event and that a time-out has been taken. A dedicated individual shall be identified for spill response. See package 1337683-71 for spill response instructions. /

4.368 Construction: PUMP grout into Spray Lance on Thermowell Riser 4 until pipe is full. /
