The information presented is as initially received without verification or evaluation and is basically all that is known by OIE staff as of 2:00 p.m. on date of this notification. Subjeg : LOSS OF SERVICE WATER TO MAIN TURBINE, FEEDWATER PUMPS, AND CONDENSATE PUMPS LUBE COOLERS At approximately 6:00 a.m. on April 1, 1976, the service water to the lube oil cooling systems for the Unit 2 main turbine, feedwater pumps, condensate pumps and heater drain pumps was isolated through an operator valving error. The operator had been instructed to isolate the service water return line to the discharge tunnel on Unit 1. -

Instead, he closed the isolation valve on Unit 2. At approximately 8:10 a.m. an instantaneous puff of fire was observed from the No. 9 bearing on the Unit 2 main generator and the unit was manually shut down. No known safety problems resulted from shutting down the

! reactor. Preliminary inspection indicates that the No. 5 turbine l i bearing is damaged due to the isolation of the turbine lube oil cooling system. .

A planned outage originally scheduled for the next week to modify

! the condensate systems for Units 1 and 2 was advanced to incorporate repairs to the turbine. No estimate on outage length is available j at this time.

The licensee is currently reviewing the incident. No press release is planned by the licensee. Region III (Chicago) is following the g j cause and corrective actions of the licensee. ,#

from an inspector on site at 3:30 p.m. on April 1, 1976. This d information is current as of 9:00 a.m. on April 2, 1976.
