SUBJECT: NRC INSPECTION REPORT 50-313/98-04; 50-368/98-04 AND NOTICE OF
VIOLATION

Dear Mr. Hutchinson:
From March 29 through May 9,1998, a routine resident inspection was conducted at your
Arkansas Nuclear One, Units 1 and 2, reactor facilities. The enclosed report presents the scope
and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the
. ANO facility was generally characterized by safety-conscious operations, sound engineering and
maintenance practices, and appropriate radiological controls.- One violation of NRC
requirements was identified. The violation resulted ' rom the improper clearance of a hold card
on a circuit breaker which supplies power to the motor operator for a Unit 1 circulating water
pump discharge valve. Although no personnel were injured, this error created the potential for a
serious personnel injury. In January 1997, a similar near-miss accident occurred when the
~ improper clearance of a hold card resulted in the energization of a 480v bus that electricians
were working in at the time. Although your corrective actions for this previous NRC violation
appeared to be comprehensive, you continue to experience errors associated with the
implementation of your hold card program. Your attention is required to ensure that your
corrective actions are effective in addressing hold card errors.
The vio[ation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation are described in detail in the enclosed report. Please note that you are
required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosure (s), and your respon'se will be placd in the NRC Public Document Room (PDR).
