Gentlemen:
This refers to the inspection conducted by Messrs. W. D. Johnson and R. C. Haag
of this office during the period June 1-30, 1989, of activities authorized by
NRC Operating Licenses DPR-51 and NPF-6 for Arkansas Nuclear One, Ur 's 1
and 2, and to the discussion of our findings with Mr. N. S. Carns and other
members of your staff at the conclusion of the inspection.
Areas examined during the inspection included followup of events, operational
safety verification, followup a previously identified items, maintenance,
surveillance, and review of contingency strike plans. Within these areas, the
inspection consisted of selective examination of procedures and representative
records, interviews with personnel, and observations by the inspectors. The
findings are documented in the enclosed inspection report.
During this inspection, it was found that certain of your activities were in
violation of HRC requirements. Consequently, you are required to respond to
these violations, in writing, in accordance with the provisions of
Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of
Federal Regulations. Your response sheuld be based on the specifics contained
in the Notice of Violation (Notice) enclosed with this letter.
In addition,
your response to Violation A should address the adequacy of tracking corrective
actions which are identified as prerequisites to plant evolutions and should
address the inspector observations regarding operations performed during the
reactor coolant system drain down evolution.
We have also examined actions you have taken with regard to previously
identified inspection findings. The status of these items is identified in
paragraph 5 of the enclosed report.
An unresolved item is identified in paragraph 6 of the enclosed inspection
report regarding the ability of degraded caulking in a seismic gap to separate
the potential effects of a high energy line break from the electrical equipment
in an adjacent room.
