other members of your staff at' the conclusion of the inspection.

Areas examined during the inspection included followup of events, operational

' safety verification, surveillance, and maintenance. Within these areas,.the

inspection. consisted of selective examination of procedures and representative

(records, interviews with personnel, and observations by the inspectors. The

findings are documented in the enclosed inspection report.

During this inspection, it was found that certairi of your activities wert: in

violation of NRC requirements. Consequently, you are required to respond to

this violation, in writing, in accordance with the provisions of Section 2.201

-of the NRC's ," Rules of Practice," Part 2, Title 10, Code of Federal

Regulations. Your response should be based on the specifics contained in the

. Notice of Violation (Notice) enclosed with this letter,

We are also concerned about the apparent reluctance of plant maintenance

. personnel to obtain engineering support / involvement when discrepancies are

.. identified duri_ng the conduct of maintenance. Such support / involvement is

. essential, from a safety stand point, to assure plant configuration control.

Two recent observations by our inspectors, documented in paragraph 7 of the

enclosed irispection report and in paragraph 3.a of NRC Inspection

Report 50-313; 368/89-28, indicate that engineering discrepancies are not

'being properly resolved. -Our concern relates to the potential for loss of

configuration control and the attendant potential for unauthorized changes to

plant safety systems and equipment by maintenance. Accordingly, we request

that you review these observations and the results of your own in-process

s

monitoring of maintenance activities by supervision and quality control / quality

assurance, to determine the reasons for the observed lack of. engineering

i

involvement and the scope of the observed condition. Your results should be

providad to this office, in writing, along with a description of those actions

!

you have taken, or planned, to improve this essential plant support interface.

to the clearance procedures of the Office of Management and Budget as required
