SUMMARY

.

Scope

-This special, announced inspection was the initial followup on Emergency

Response Facility . Appraisal (ERFA) findings identified during the ERFA

conducted June l27-30, 1988 (50-338, 339/88-14).

Inspection included a detailed

l

review, evaluation, and discussion of the licensee's response to the ERFA open

l

-1tems and Emergency Preparedness Program enhancement items.

A review of all

l.

outstanding- nonapprai. sal emergency preparedness findings and the corrective

p

actions implemented in response thereto was performed (Paragraph 4).

,

Results-

A detailed review, evaluation, and discussion of the licensee's current ERFA

Action Plan disclosed that corrective actions- for only two ERFA open items

(88-14-05,88-14-07) were satisfactorily completed (Paragraph 2).

It was also

disclosed that corrective actions for the major open items and related Emergency

Preparedness Program enhancement items addressing dose assessment / projection,

-and their respective computerized and manual calculational methods, were not

implemented (88-14-01,.88-14-02, 88-14-03, G8-14-04). The projected corrective

a'ction completion date of July 31, 1989, was assigned for these items.

The

apparent significant delay in implementing prompt corrective action in response

to :the appraisal findings was fully discussed with cognizant licensee

representatives prior to and during the exit interview (Paragraph 5).

Evaluation of three additional Emergency Preparedness items unrelated to the

appraisal, disclosed that acceptable corrective actions were completed for the

violation addressing use of an updated emergency response personnel

augmentation telephone list (50-338, 339/88-24-01), and an IFl regarding

Emergency Preparedness drill critique reports (f.-' 338, 339/88-24-03).

The

third item (50-338, 339/88-24-02), which involved a violation regarding

staffing requirements for Emergency Response Team personnel, remained open

pending an NRC review and assessment of a licensee proposed Emergency Plan

Revision addressing this subject (Paragraph 4),

Licensee Action on ERF~ Appraisal Open Items (92701)

This section addresses the status of the licensee's response to ERFA Open

Items identified during the ERF Appraisal performed June 27-30, 1988

(50-338,339/88-14). Note that prompt corrective action was not completed

for those items addressing dose assessment / projection (88-14-01, 88-14-02,

88-14-03, 88-14-04).

Inspection disclosed that the licensee's Appraisal-

Action plan, dated November 21, 1988, extended the projected corrective

action completion date for these items from January / February 1,1989

to July 31, 1989.

Based on this projection, the total elapsed time

between identification of the subject items and projected completion of

correction of same will approach 13 months. The subject delay represents

a significant departure from expected prompt implementation and completion

of effective corrective actions.

The subject open items and related

Emergency Preparedness Program enhancement items were fully discussed with

cognizant licensee representatives prior to and during the inspection exit

interview.

All referenced items will remain open pending acceptable

resolution.

(0 pen) ERFA Open Item S0-338, 339/88-14-01:

Completion of resolution

of differences in dose calculations between the RAD / MET Model and

manual method defined in the Emergency Plan Implementing Procedures

(EPIPs).

Inspection disclosed that software changes to the RAD / MET model

(Class A Dose Assessment Model) necessary to resolve differences

between the manual EPIP method and the computerized RAD / MET model

were submitted to tha licensee's Power Engineering Services on

December 21, 1988.

The respective memorandum indicated that

approximately six weeks would be recessary to implement, document and

test the necessary changes.

The referenced Appraisal Action Plan

reassigned a corrective action completion date of July 31, 1989.

This item remains open until a satisfactory corrective action is

completed.

(0 pen) ERFA Open Item 50-338, 339/88-14-02:

Completion of evaluation

of validity of use of field monitoring data for calculating a source

term and use of same in the protective action decision process, or in

determining emergency classifications (50-338, 339/88-14-02).

Based on the recognized non-conservative calculation methodology

involved, the licensee deleted this capability from the RADMET model

and EPIP-4.13 (Offsite Release Assessment with Environmental Data).

The changes will be coordinated with a major revision to the manual

calculation procedures, and the RADMET computer code. The projected

completion date for proposed corrective action was July 31, 1989,

(0 pen) ERFA Open Item 50-338, 339/88-14-03:

Completion of revision

to the EPIPs addressing dose assessment to i vlude separate stability-

classes for vertical and horizontal diffusion.

Inspection disclosed that the licensee will revise EPIP-4.10

(Determination of X/Q) to include the Pasquill-Gifford-Turner

stability class tables in lieu of implementing a system of separate

stability classes for vertical and horizontal diffusion. This change

> ss planned to be implemented concurrent with the above cited major

/ision of the manual dose calculation procedurec.

The Appraisal

...: tion Plan listed a completion date of July 31, 1989.

Inspection

disc 1csed, however, that existing methodologies presently used by the

licensee are consistent with NRC regulations and guidance.

This item will remain open until the committed procedural changes

have been implemented and determined to be adequate.

(0 pen) ERFA Open Item 50-338, 339/88-14-04:

Completion of

modification of the RADMET model to provide dose projection estimates

at future plume positions.

Inspection disclosed that the licensee planned a modification to the

RADMET Computer Model to provide dose projection estimates as one of

the code enhancements.

This modification will be made concurrent

with the previously referenced computer code changes. The projected

completion date of July 31, 1989 was assigned,

of TSC ventilation operation procedure (1-0P-21-10) to include

operator verification or normal system damper positions to ensure

proper system operation.

Completion of revision

of TSC ventilation test prucedure (1-PT-77.9) to test system

components, including system interlocks.

Inspection disclosed that the licensee determined that the electrical

schematics of the TSC heating, ventilation, and air conditioning

(HVAC) system were inaccurate.

Correction of this item was

reassigned to the Station Design Engineering Department ' for

assessment and resolution.

Following correction of the schematics

(scheduled for completion during March 1989), the licensee committed

to revise the test procedure and initiate tests of the system

interlocks and remote indicators.

(Closed) ERFA Open Item 50-338, 339/88-14-07:

Completion of

establishment of a mechanism to track all sensor data errors and

assist in correction and prevention of such errors.

Inspection disclosed that the ERF computer system used an ERF

database program to track all system problems.

The database was

expanded to include tracking of SPDS sensor data errors.

Additionally, the ERF Computer System Coordinator is responsible for

tracking the progress of corrective actions addressing sensor data

errors and ensuring that they are entered into the database following

resolution of same.

The system generated reports to determine

problem areas and to establish specific trends.

Response to this

item was determined to be adequate and complete.

Status Review of ERF Appraisal Emergency Program Enhancement Items (97201)

This section addresses the status of the licensee's response to Emergency

Program Enhancement Items identified and discussed in detail with

cognizant licensee representatives during the ERFA performed June 27-30,

1988.

Enhancement items ara emergency program improvement areas

identified by the inspector.

The licensee committed to evaluate and

consider these items for implementation and/or correction, as indicated,

(Closed) ERFA Enhancement Item:

Modifying the manual EPIP dose

calculation method to run on a personal computer or a computer system

separate from RADMET.

This computerized EPIP method could be made

available in the Control Room, TSC, LE0F, and CEOF.

Inspection disclosed that the licensee was preparing to revise the

manual dose assessment procedures to achieve the following:

(1) eliminate any differences that exist with the RADMET computer

code; (2) eliminate procedural redundancy; and (3) provide simple and

user-friendly instructions. The licensee projected a completion date

of July 31, 1989.

Conducting a more detailed

comparison between RADMET and the Commonwealth of Virginia's dose

assessment model following completion of the Commonwealth's proposed

modifications of the model.

Reasons should be identified if any

comparisons differ by more than a factor of 3.

Resolution of the subject item is contingent upon completion of

modification of the RADMET computer code addressed in ERFA Open

Item 50-338, 339/88-14-02, above. Projected completion date assigned

was July 31, 1989. Based upon its close relationship, this item will

be tracked as an adjunct to the referenced ERFA item. This item is

closed as a separate entity solely on this basis, and should be

addressed in the subject ERFA open item corrective action.

(Closed) ERFA Enhancement Item:

Developing a version of the RADMET

model with a domain that extends through the ingestion pathway zone.

The licensee can use the GASPAR model provided via the Corporate

mainframe computer to perform ingestion pathway calculations. Access

to the respective code is available onsite.

(Closed) ERFA Enhancement Item:

Providing support for a backup

method for estimating the vertical diffusion coefficient that does

not involve sigma theta.

Two possible alternates for this support

are an independent, backup delta T system, end implementation of a

procedure to estimate stability class from solar radiction and wind

speed.

The proposed corrective action for ERFA Open Item 88-14-03 elso

I

addresses- the subject Enhancement Item.

In response to this open

item, the licensee committed to revise the manual dose assessment

procedures to include the Pasquill-Gifford-Turner tables as a backup

method for determining stability class.

Based upon its close

relationship, the subject enhancement item will be factored into the

preposed corrective action of the referenced ERFA open item.

Protecting the meteorological tower

licensee assigned a completion date of March 15, 1989.

Inspection disclosed that the licensee intiatied required plans to

date of April 1 - 1989.

Resolution of the subject item will be

tracked as IFI 50-338, 339/89-02-02.

(0 pen) ERFA Enhanessnt Item:

Developing a procedure to test the

throw-over featura at the UPS buss.

Inspectio.. disclosed that the sutiject item was tracked on the ERFA

Action Plan.

No action regarding this item was reported; however,

the Action Plan assigned a completion date of April 1,1989.

This

item will be tracked as IFI 50-338, 339/89-02-03 until such time that

the commitment is resolved.

(0 pen) ERFA Enhancement Item:

Providing a preventive maintenance

(PM) program for key emergency ventilation system components.

The HVAC system walkdown was conducted to identify improperly labeled

equipment and provide local labeling of damper positions.

Additionally, a system evaluation for identifying other components

which needed to be included in the PM program were complete for the

TSC; however, the subject evaluation did not appear to be fully

completed for the LE0F.

This item will be tracked as IFI 50-338,

339/89-02-04.

(Closed) ERFA Enhancement Item:

Revising the non-safety preventive

maintenance (PM) program to ensure periodic maintenance of non-safety

system and documentation thereof.

Inspection disclosed that modifications to the non-safety PM program

were implemented to ensure coverage of ERF systems as cited in

Item h, above.

It was questionable, however, whether the LE0F HVAC

system was adequately included by the PM program. This item will be

addressed and tr6cked under Item h, above. Accordingly, this item is

closed as a separate entity solely on this basis.

Documenting periodic testing of

non-safety systems and the results thereof.

The PM testing program was expressly designed, and implemented to

evaluation this item and determined that it was not feasible to add

the suggested capability to the existing SPDS.

Status of Previous Non-ERFA Findings.(92701)

a.

(Closed) Violation 50-338, 339/88-24-01:

Failure to use most recent

revision of emergency personnel augmentation telephone call list.

The subject finding addresses failure of emergency personnel to

follow the requirements of Station Administrative Procedure ADM-6.4

via use of an uncontrolled, outdated revision of Section 1.2 of the

Station Augmentation Telephone Directory.

Inspection disclosed that

corrective action included the following:

(1) issuance of a memo by

the Station Manager to all

personnel

regarding required

implementation of Procedure ADM-6.4, which governs the use,

reproduction, storage, and availability of controlled documents;

(2) assurance that cognizant security personnel responsible for

initiatica of Emergency Response Augmentation Call-out obtain and

maintain updated controlled copies of Section 1.2 of the Station

Emergency Telephone Directory.

The corrective action was determined

to be adequate,

b.

(0 pen) Violation 50-338, 339/88-24-02:

Failure to demonstrate the

capability of meeting staffing requirements for emergency

preparedness reaponse team personnel.

The licensee submitted to the NRC a proposed revision of Tables 5.1

and 5.2 of Section 5 of the Station Emergency Plan addressing

emergency staff augmentation.

Resoluation of this item remains open

pending NRC review of the licensee's referenced submittal.

c.

(Closed) IFI 50-338, 339/88-24-03:

Incorporate a statement in the

drill report addressing whether drill objectives were met or same

were not met.

Develop an action plan to setisfy the correction.

Inspection disclosed that reports for drills conducted subsequent to

the subject finding addressed in detail drill objectives and the

status thereof. An adequate statement was included.

5.

Exit Interview

The inspection scope and findings were summarized on February 23, 1989,

with those persons indicated in Paragraph 1, above.

described the areas evaluated and discussed in detail each ERFA open and

enhancement

item identified during the ERFA conducted in June 1988.

Enhancement items tracked by the licensee and assigned a projected
