Dear Mr. Gipson:

On April 9,1998, the NRC completed an inspection at your Enrico Fermi, Unit 2 facility. The
enclosed report presents the results of that inspection.
The inspection was an examination of activities conducted under your license as they relate to
l
radiation safety and to compliance with the Commission's rules and regulations and with the
conditions of your license. The purpose of this inspection was to review the control of high'
L
radiation areas; radiological surveys, labels, and postings; control of radiological conditions
within the plant; self-assessments and audits; and radiation protection coverage of tasks to
maintain radiation dose As-Low-As-is-Reasonably-Achievable (ALARA). Within these areas,
,

the inspection consisted of a selective examination of procedures and representative records,
l
observations of activities, observations of equipment materiel condition, and interviews with
l
personnel.
Overall, your staff effectively controlled access to high radiatica areas, radiation dose, and
radiological conditions for work within the Radiologically Restricted Area, in particular,
contaminated areas and hot spots within your facility were kept to a minimum and permanent -
.
shielding had reduced dose rates from hot spots in high traffic areas. Radiological surveys,

,
labels, and postings were informative and well documented, and no materiel condition or
L
housekeeping issues were identified. Self-assessments and audits were sufficiently critical and
issues were addressed appropriately.
,
However, two examples of a violation of Radiation Work Permit (RWP) requirements were
identified. These examples involved: (1) workers entering a locked high radiation area without
the required stay time tracking for dose control; and (2) a worker breached a potentially
,

contaminated system without the required radiation protection coverage.- Although these
,
occurrences did not result in significant radiation doses, we are concemed that both examples
'
involved mechanical maintenance staff and that these occurrences constituted failures to follow

. basic, long-standing radiation protection requirements. This violation is cited in the enclosed

Notice of Violation (Notice), and the circumstances surrounding it are described in detail in the
