Gentlemen:
This refers to the inspection conducted by Messrs. P. C. Wagner, L. D. Gilbert,
and M. E. Murphy of this effice during the period April 17-21, 1989, of
activities authorized by NRC Operating Licenses DRP-51 and NPF-6 for Arkansas
Nuclear One, Units 1 and 2.
Discussions of our findings were held with
Mr. J. Levine and other members of'your staff during and at the conclusion of
the inspection.
Areas examined during the inspection included the actions taken to implement an
integrated corrective cctico program and the response to the Unit 2 extraction
.
steam line break which occurred on April 18, 1989. Within these areas, the
inspection consisted of selective examination of procedures and representative
'
records, interviews with personnel, and observations by the NRC inspectors.
The inspection findings are documented in the enclosed inspection report.
On April 7, 1989, the NRC issued a Notice of Violation and Proposed Imposition
of Civil Penalties related to activities at your facility. One of the
violations addressed your failure to take prompt corrective action once
problems are identified. No violations or deviations are included in this
inspection report. However, the NRC inspectors determined that there was a
timeliness problem with the implementation of the new corrective action program
and noted problems with the completion of the controlling procedures. The NRC
inspectors also discussed the advisability of implementing an aggressive
trending program for identified problems and their resolution.
No separate
response is requested with respect to this inspection report because these
items should be encompassed in your response to .the previous Notice of
Violation and will be followed up in the NRC review of your response to the
previous Notice of Violation.
