The enclosed report transmits the findings of the April 17-27, 1989, . special
inspection' concerning two ' operational events and the installation of an unre-
viewed temporary modification at the. Calvert Cliffs facility.
These findings

were discussed with Mr. L. Russell and members of your staff.
On. April 17,.1989, core alterations were performed for approximately 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and

35. minutes without the appropriate level of containment integrity being - estab-
~11shed. Again on April 19, 1989, core alterations were performed for approxi-
mately 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> while the containment bounda y was breached. Technical Specifi-
cations (TS) impose special containment integrity requirements during core
alterations.
These . two operational events therefore represent an apparent

violation.-

On April 22, 1988, a temporary modification was installed on_the Unit 1 Refuel-
ing Machine which your staff subsequently, on April 20, 1989 determined consti-
l
tuted.an unreviewed safety question. This temporary modification was installed
l
without the TS required Plant Onsite Safety Committee review or an evaluation
!
performed as required by 10 CFR 50.59.
This constitutes two apparent
violations.
Of more significant concern to us are the broader implications of these
-events.
The NRC is concerned that a lack of control of operations and main-
. ,
tenance activities at Calvert Cliffs allowed these events to occur.

Accordingly, we have scheduled an Enforcement Conference on May 30,1989, to

discuss these events.with you in the NRC Region I Office in King of Prussia,
!
We will forward an appropriate notification of enforcement
i
action after the Enforcement Conference.
