t ENCLOSURE 1 NOTICE OF VIOLATION Florida Power Corporation Docket No.
50-302-Crystal River'L51t 3 License No. DPR-72 During the Nuclear Regulatory Commission (NRC) inspection conducted on July 8 -
August 4', 1989, violations of.NRC requirements were identified. In accordance
.with the' " General, Statement of' Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violations are listed below:
A.
Technical Specification (TS) 4.6.4.1 requires that each containment
' hydrogen monitor.be demonstrated operable by the performance of a channel functional test at least once per 31 days.
This TS required functional test became due on June 7, 1989, 31 days after the implementing procedure was issued on May 8, 1989.
Contrary:.to the above, as of July 13, 1989, a-channel functional test has not been performed on the containment hydrogen monitors (WS-10-CE and WS-11-CE).
This is a Severity Level IV violation (Supplement I).
B.
TS 4.6.1.1.a.1 requires in a part that primary containment integrity be demonstrated at least once every 31 days by verifying that all containment penetrations required to be closed during accident conditions and not capable of being closed by automatic isulation valves, are closed by-valves, blind flanges, or deactivated automatic valves secured in their positions.
Contrary to the above, as of July 24, 1989, six manual vaives associated with emergency feedwater containment penetrations #109 and #424 were not verified closed every 31 days.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Florida Power Corporation is hereby
. required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC 20555, with a copy to the. Regional Administrator, Region II, and a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a " Reply to a Notice of Violation" and should include [for.each violation]:
(1) admission or denial of the violation, q
(2) the' reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will l
be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.

time specified. in this Notice, an order may be issued to show cause why. the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
