SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/89-09 AND 50-328/89-09)

This refers to the Nuclear Regulatory Commission (NRC) routine monthly

inspection conducted- by K. Jenison from March 5,1989 thru April 5,1989. At

the conclusion of the inspection, the findings were discussed with tho.se

members of your staff identified in the enclosed inspection report.

Areas. examined during the inspection are identified in the report. These ar.eas

included operational safety verification, refueling activities, maintenance and

surveillance activities. Within these areas, the inspection consisted of

selective examinations of procedures and representative records, interviews

with personnel, and observation of activities in progress.

The inspection' findings indicate that certain activities . appeared to violate

NRC requirements. The violation involved three examples of procedural

inadequacies. The third example of the violation is similar to a vendor manual

problem described in URI 327,323/88-50-07. You are requested to address these

two vendor manual problems collectively in your response to the third example

of the violation indicating whether they involve a programmatic weakness in the

vendor manual control and validation program.. Additionally, since these

problems involve the interface between your plant and engineering organize--

tions, you are requested to include in your response an evaluation as tu

whether adequate feedback between these organizations exists in relation to

the acceptability of deliverables, such as vendor manuals, to ensure that

deliverables are correct, usable, and maintained as such.

The violation, references to pertinent requirements, and elements to be

included in your response are described in the enclosed Notice of Violation.

The enclosed Inspection Report also identifies activities that appeared to

violate NRC requirements but are not cited; therefore, no response is required.

Unresolved items concerning operability of fan motors for certain ECCS room

coolers and operability of a UHI valve with scaffolding interference were

identified. These matters are currently under review by the licensee as well

as NRC and will be pursued by the NRC staff during future inspections.
