Response to Inspection Report
50-313/88-35 and 50-368/88-35
Dear Mr. Milhoan:
NRC's letter of May 23, 1989 (0CNA058922), requested supplemental
information regarding our response to the subject inspection report.
Specifically, additional corrective steps to preclude recurrence of
valve misalignments associated with a Containment Integrated Leak Rate
Test (CILRT) were to be further addressed.
As discussed with Messrs. W. Seidle and R. Azua of NRC Region IV and Messrs.
J. Vandergrift, D. Lomax, and A. Cox of AP&L on May 17, 1989, (and as

discussed in the AP&L/NRC Enforcement Conference on January 23, 1989)
actions had already been identified by AP&L that addressed concerns relating
to sign-offs on CILRT valve alignment tags to confirm " hands on" independent
verification.
In addition to the actions described in our previous response
(OCAN648905), we will include a required independent verification sign-off
on the CILRT valve tags.
An operator will perform the initial CILRT valve
alignment in accordance with the procedure, hang the CILRT valve tags, and
sign-off (i.e., initial) the valve alignment procedure forms.
A second
operator will perform an independent verification of the CILRT valve
alignment, sign-off the CILRT valve tags, and sign-off the valve alignment
procedure forms.

While we continue to believe that the valve misalignment discussed in

Notice of Violation 50-313/8835-01 occurred after the initial (and

independently verified) valve alignment, the additional action of

valve' tags will provide even more assurance that the CILRT valves are
correctly aligned for performance of the test.
