-(INSPECTION REPORT NOS. 50-424/89-10 AND 50-425/89-12)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
Messrs. J. F. Rogge, C. W. Burger, and R. F. Aiello, on February 18 - March 17,
1989.
The inspection included a review of activities authorind for your
Vogtle facility.
At the conclusion of the inspection, the firJings were
discussed with those members of your staff identified in the enclosed
Inspection Report.
NRC concerns relative to the inspection findings regarding
the misalignment of residual heat removal system valves were also discussed in
an Enforcement Conference held on March 22, 1989.
A summary of this
Enforcement Conference was sent to you on April 5, 1989.
Areas examined during the inspection are identified in the report.
Within

these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
i
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The first violation described in the enclosed Notice of
Violation involved the misalignment of residual heat removal system valves on
March 9,1989, which rendered two Emergency Core Cooling System subsystems

This violation was discussed in the above referenced Enforcement
Conference.
Although we considered classification of this violation as a
Severity Level III, it has been classified as a Severity Level IV based on the

minimized safety significance of this event in that Unit 2 had not been
critical and had not generated any decay heat. However, we emphasize that any
j
similar violation in the future may result in escalated enforcement action.

The second violation described in the enclosed Notice has also been categorized
as a Severity Level IV violation.
This violation was not discussed at the
above referenced Enforcement Conference.
These violations are further
described in the enclosed Notice of Violation and Inspection Report; also
delineated therein are references to pertinent requirements and elements to be
addressed in your response.

In addition to the need for corrective action regarding the specific matters
identified in the enclosed Notice, we are particularly concerned that operations
supervision directed that locked safety-related valves be manipulated without
the benefit of approved procedures. Continued management attention is necessary
to ensure the proper attitude of the operations staff towards safety.
Addi-
tionally, we are concerned that the operating experience level may not be
balanced on each operations shift.
This is particularly important as Unit 2
becomes operational in order to ensure that the large number of safety system
challenges that occurred on Unit 1 is not repeated for Unit 2.
Consequently,
your response to violation A should address these concerns.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosures are not subject to the
clearance procedures of the Office of Management and Budget as required by the
Paperwork Reduction Act of 1980, Pub. L. No.96-511.
