,Jf:r 8.0 RECORDS AND REPORTS 8.1 RECORDS OF INCIDENTS Incident reports for the record will be made for " unusual" events and the more-serious event classifications (see 3.0).

1he preparation of these reports will be-the responsibility of the facility / operational management of the involved facility..The report will include the cause of the incident, personnel and/or er4uipment involved, extent of injury and/or damage, correc-Live actions' taken during the emergency, recovery steps taken to return to operation, and the actions taken to prevent recurrence.

Participation and evaluation of on-site and off-site support assistance, with recommendations (if any) for Improvements, will be included.

The records will be maintained

' and retained by the Director, Nuclear Safety & l.icensirigf as long as the facility / operation is active in licensed progrrms.

8.2. RECORDS OF PREPAREDNESS ASSURANCE Records 'will be maintained for all drills and tests of the plan. These records will include the reports of observers and their recommendation for

linprovement. These records, along with inventory records of. emergency sup-plies are maintained by Industrial Security for_a period of 2 years (reference 4.2.2.3).

Agreement letters for of f-site support organizations will be re-tained by Industrial Security for at least 3 years following a reduction in SNM pcssession such that a Radiological Contingency Plan is no longer re-

' quired. Maintenance, testing, and calibration records on equipment required for emergency response (whether stored or in active use) are maintained by the CRIS-system as described in Subsection 7.5.

Records of training, as required by Subsection 7.2, are maintained by Technical and Skills Development.

These records are retained on site for a minimum of 2 years beyond which time they are submitted to long-term storage (75 years) off site.
